中药芪连清胃汤剂辅助治疗幽门螺杆菌阳性胃溃疡的效果及对炎症因子水平的影响

摘　　要：目的 探讨在常规西药四联疗法的基础上辅助使用中药芪连清胃汤剂治疗幽门螺杆菌(Hp)阳性胃溃疡的临床效果。方法 选取2020年3月至2023年3月大同大学附属医院消化内科收治的Hp阳性胃溃疡患者116例作为研究对象,根据治疗方案不同分为观察组(n=56)和对照组(n=60)。对照组使用四联方案,观察组在对照组基础上增加使用中药芪连清胃汤剂,比较两组的治疗效果。结果 两组治疗前胃脘痛、恶心呕吐、反酸嗳气、纳差证候评分比较差异无统计学意义(P>0.05);治疗后观察组胃脘痛、反酸嗳气、纳差证候评分低于对照组(P<0.05)。两组治疗前炎症细胞浸润、黏膜厚度、黏膜形态评分比较差异无统计学意义(P>0.05);治疗后观察组黏膜厚度、黏膜形态评分低于对照组(P<0.05)。观察组治疗有效率和Hp转阴率均高于对照组,但差异无统计学意义(P>0.05)。两组治疗前白细胞介素-2(IL-2)和白细胞介素-12(IL-12)水平比较差异无统计学意义(P>0.05);治疗后观察组IL-2和IL-12水平低于对照组(P<0.05)。结论 在Hp阳性胃溃疡的治疗中加入中药芪连清胃汤可取得更理想的治疗效果。Objective To explore the clinical effect of Qilianqingwei Decoction on Helicobacter pylori(Hp) positive gastric ulcer on the basis of conventional western medicine quadruple therapy.Methods A total of 116 patients with HP-positive gastric ulcer admitted to the Department of Gastroenterology,Affiliated Hospital of Datong University from March 2020 to March 2023 were selected as the study objects,and divided into observation group(n=56) and control group(n=60) according to different treatment regimens.The control group was treated with four combinations,and the observation group was treated with Qilianqingwei Decoction on the basis of the control group.The therapeutic effect of the two groups was compared.Results There were no significant differences in the scores of epigastric pain,nausea and vomiting,acid belching and poor appetite syndrome between the two groups before treatment(P>0.05).After treatment,the syndrome scores of epigastric pain,acid belching and poor appetite in the observation group were lower than those in the control group(P<0.05).There were no significant differences in inflammatory cell infiltration,mucosal thickness and mucosal morphology scores between the two groups before treatment(P>0.05).After treatment,the mucosal thickness and mucosal morphology scores of observation group were lower than those of control group(P<0.05).The treatment effective rate and Hp negative conversion rate in the observation group were higher than those in the control group,but the difference was not statistically significant(P>0.05).There was no significant difference in the levels of interleukin-2(IL-2) and interleukin-12(IL-12) between the two groups before treatment(P>0.05).After treatment,the levels of IL-2 and IL-12in observation group were lower than those in control group(P<0.05).Conclusion Adding Qilian Qingwei decoction to the treatment of Hp-positive gastric ulcer can achieve more ideal therapeutic effect.
