贵州省农村地区老年人代谢肥胖表型与多维衰弱相关性分析

摘  要:目的探讨贵州老年人代谢肥胖表型与多维衰弱的相关性,为预防老年人生活质量下降和促进老年人健康提供参考依据。方法2019年7—8月,采用横断面研究,抽取贵州省黔南州贵定县和贵阳市花溪区≥60岁的1116名老年人为研究对象,根据体质量指数(body mass index,BMI)判断超重肥胖、代谢是否正常将老年人分为四种代谢表型,采用TilBurg衰弱量表来衡量老年人的多维衰弱情况,分析不同表型肥胖与多维衰弱及其不同维度的相关性。结果多因素logistic回归显示,调整潜在混杂因素后,以正常代谢体质量正常者(metabolic healthy normal weight,MHNW)为参照组,异常代谢超重肥胖(metabolic abnormal obesity,MAO)者和正常代谢超重肥胖(metabolic normal obesity,MHO)者的多维衰弱患病风险增高,比值比(odds ratio,OR)分别为(OR=1.66,95%CI:1.03~2.67)和(OR=1.53,95%CI:1.03~2.28),而MANW与MHNW多维衰弱比较差异无统计学意义(OR=1.34,95%CI:0.87~2.07,P=0.189)。Logistic回归分析显示,在调整潜在混杂因素后,MHO人群身体衰弱患病率为MHNW的1.72倍(OR=1.72,95%CI:1.01~2.23,P=0.044),而心理衰弱(OR=1.12,95%CI:0.56~2.26,P=0.454)、社会衰弱(OR=1.53,95%CI:0.77~3.04,P=0.229)与不同代谢肥胖表型间均无相关性。结论老年人代谢肥胖表型中MHO与MAO表型可增加其多维衰弱的风险,MHO增加身体衰弱的风险,但代谢肥胖与多维衰弱之间的关系还需进一步证实。Objective To investigate the association between different metabolic obesity phenotypes and multidimensional frailty(MF)in elderly individuals in Guizhou and to provide a reference basis for preventing the decline in quality of life and promoting health in the elderly.Methods In a cross-sectional study design conducted in July-August 2019,1116 elderly individuals aged≥60 years from Guiding County,in Qian'nan Prefecture and Huaxi District in Guiyang City,Guizhou Province,were selected as the study subjects.Based on the combination of body mass index(BMI)indicating overweight or obesity and metabolic status,four metabolic obesity phenotypes were classified.The elderly individuals were classified into phenotype categories,and the Tilburg Frailty Indicator was used to measure multidimensional frailty.The correlation between different obesity phenotypes and multidimensional frailty and its different dimensions was analyzed.Results Multivariate logistic regression analysis was performed that,after adjusting for potential confounding factors,metabolic abnormal obesity(MAO)and Metabolic normal obesity(MHO)individuals had an increased risk of multidimensional frailty,odds ratio(OR)are 1.66(95%CI:1.03-2.67)and 1.53(95%CI:1.03-2.28)respectively,with metabolic healthy normal weight(MHNW)as the reference;the difference between the metabolic abnormal normal weight(MANW)group was not statistically significant as the reference(OR=1.34,95%CI:0.87-2.07,P=0.189).Further logistic regression of the different dimensions of multidimensional frailty showed that after adjusting for potential confounders,the prevalence of physical frailty in the MHO population was 1.72 times higher than normal(OR=1.72,95%CI:1.01-2.23,P=0.044)while there was no statistically significant association between psychological frailty,social frailty and the different metabolic obesity phenotypes(OR=1.12,95%CI:0.56-2.26,P=0.454;OR=1.53,95%CI:0.77-3.04,P=0.229).Conclusions The MHO and MAO phenotypes in metabolic obesity increase their risk of multidimensional fra
