摘　　要：目的探究分化型甲状腺癌(DTC)患者术后,首次^(131)I治疗后全身扫描(Rx-WBS)时肝弥漫性摄取(DHU)的影响因素。方法本研究为回顾性病例对照研究,选取2021年6月至2023年8月宝鸡市中心医院核医学科收治的104例DTC患者,男24例,女80例,年龄(48.16±13.28)岁,年龄范围为16~79岁。采用单因素分析及多元有序logistic回归分析筛查影响DHU水平的影响因素。结果根据Rx-WBS中DHU水平分为DHU 1组(n=24)、DHU 2组(n=57)、DHU 3组(n=23)。DHU 1组甘油三酯[2.2(1.5,3.2)mmol/L]、DHU 2组甘油三酯[1.9(1.5,2.9)mmol/L]、DHU 3组甘油三酯[1.4(1.2,1.8)mmol/L]比较,差异有统计学意义(P<0.05)。DHU 1组低密度脂蛋白[(4.1±1.2)mmol/L]、DHU 2组低密度脂蛋白[(3.7±0.9)mmol/L]、DHU 3组低密度脂蛋白[(3.7±1.2)mmol/L]比较,差异有统计学意义(P<0.05)。DHU 1组肺转移[0%(0/24)]、DHU 2组肺转移[1.8%(1/57)]、DHU 3组肺转移[13.0%(3/23)]比较,差异有统计学意义(P<0.05)。肺转移是影响DHU水平的独立影响因素(OR=0.064,95%CI:0.004~0.888,P=0.041)。结论肺转移是DHU水平的独立影响因素,并具有显著正向影响。Objective To explore the influential factors of diffuse hepatic uptake(DHU)in patients with differentiated thyroid carcinoma(DTC)after first 131I post-therapy whole body scan(Rx-WBS).Methods This study was a retrospective case-control study,a total of 104 patients with DTC admitted to the department of Nuclear Medicine in Baoji Central Hospital from June 2021 to August 2023,including 24 males and 80 females,aged(48.16±13.28)years old,ranging from 16 to 79 years old.Univariate analysis and multivariate ordered logistic regression analysis were used to analyze the screening for factors affecting DHU levels.Results According to the DHU level in Rx-WBS,the patients were divided into DHU 1 group(n=24),DHU 2 group(n=57)and DHU 3 group(n=23).The triglyceride in DHU 1 group[2.2(1.5,3.2)mmol/L],DHU 2 group[1.9(1.5,2.9)mmol/L]and DHU 3 group[1.4(1.2,1.8)mmol/L]were compared,and the differences were statistically significant(P<0.05).The low density lipoprotein in DHU 1 group[(4.1±1.2)mmol/L],DHU 2 group[(3.7±0.9)mmol/L]and DHU 3 group[(3.7±1.2)mmol/L]were compared,and the differences were statistically significant(P<0.05).Lung metastasis in DHU 1 group[0%(0/24)],DHU 2 group[1.8%(1/57)]and DHU 3 group[13.0%(3/23)]were compared,and the differences were statistically significant(P<0.05).Lung metastasis was an independent factor affecting DHU level(OR=0.064,95%CI:0.004 to 0.888,P=0.041).Conclusions Lung metastasis is an independent factor affecting the level of DHU and has a significant positive effect.
