摘　　要：[目的]探讨原发性胆汁性胆管炎(PBC)患者在不同组织学分期的肝脏脂肪变性程度和血脂特征。[方法]选取行肝脏穿刺活检的PBC患者108例和同期健康体检者57例,应用受控衰减参数值(CAP)评估肝脏的脂肪变性程度,同时分析不同组织学分期的血脂变化情况。[结果]PBC患者的CAP值低于健康人群,不随组织学分期增加而变化。三酰甘油、总胆固醇、低密度脂蛋白胆固醇和高密度脂蛋白胆固醇水平在PBC的Ⅱ期或Ⅲ期达高峰,终末期(Ⅳ期)下降。[结论]PBC患者的血脂异常比例虽高,但肝脏脂肪变性程度低于健康人群,且发现PBC患者的肝脏脂肪变性程度减低与高密度脂蛋白胆固醇水平升高相关。[Objective] To analyze the degree of hepatic steatosis and the characteristics of lipid in patients with primary biliary cholangitis(PBC)at different histological stages.[Methods] A total of 108 PBC patients and 57 healthy subjects underiwent liver biopsy were selected.Controlled attenuation parameter(CAP)values to assess the degree of liver steatosis,and analyze the changes of lipid in different histological stages.[Results]The degree of liver steatosis in PBC patients was lower than health people and didn't change with the increasing histological stages.The level of TG,TC,LDL-C and HDL-C in PBC patients were elevated to a peak in stage Ⅱor Ⅲ and decreased in terminal stage(stage IV).[Conclusion] This study confirmed that although PBC patients have a high proportion of dyslipidemia,but the degree of liver steatosis was lower than health people,and we found the correlation between low liver steatosis and high levels of PBC patients.
