自拟疏肝理气解郁汤对甲状腺癌术后患者的疗效观察

摘　　要：目的 探讨自拟疏肝理气解郁汤对甲状腺癌术后患者的疗效。方法 选取甲状腺癌术后患者85例,中医辨证为气郁痰阻证,随机数字表法分为研究组43例,对照组42例。对照组予以常规治疗(口服左甲状腺素钠片,并进行心理及健康行为指导),研究组在对照组基础上予以自拟疏肝理气解郁汤治疗,2组均治疗3个月。统计2组中医证候疗效,比较治疗前后中医证候积分、负性情绪[21项抑郁-焦虑-压力量表(DASS-21)评分]、免疫功能(CD4^(+)、CD8^(+)、CD3^(+)水平变化)。结果 研究期间研究组依从性差剔除1例,纳入42例;对照组自行退出1例,纳入41例。治疗后研究组主症、次症及舌脉积分低于对照组(P<0.05);治疗后研究组中医证候疗效总有效率(90.48%)高于对照组(73.17%)(P<0.05);治疗后研究组DASS-21评分低于对照组(P<0.05);治疗后研究组CD4^(+)、CD3^(+)水平高于对照组,CD8^(+)水平低于对照组(P<0.05)。结论 自拟疏肝理气解郁汤治疗甲状腺癌术后患者,可明显缓解患者临床症状,减轻负性情绪,调节免疫功能,提高临床疗效。Objective To investigate the effect of self-designed Shugan Liqi Jieyu Decoction in patients with thyroid carcinoma after operation.Methods A total of 85 patients with thyroid carcinoma after operation were selected,and the syndrome differentiation of TCM was Qi Stagnation and Phlegm Obstruction.They were randomly divided into the study group(43 cases)and the control group(42 cases).The control group was given conventional treatment(oral levothyroxine sodium tablets,and psychological and health behavior guidance),while the study group was given self-designed Shugan Liqi Jieyu Decoction on the basis of the control group.Both groups were treated for 3 months.The TCM syndrome efficacy of the two groups was statistically analyzed,and TCM syndrome score,negative emotion[score of depression anxiety stress scale-21(DASS-21)]and immune function(changes in CD4^(+),CD8^(+)and CD3^(+)levels)were compared before and after treatment.Results During the study period,1 case of poor compliance was excluded and 42 cases were included.In the control group,1 patient withdrew spontaneously and 41 patients were included.After treatment,the scores of main disease,secondary disease and tongue and pulse in the study group were lower than those in the control group(P<0.05).After treatment,the total effective rate of TCM syndrome in the study group was 90.48%higher than that in the control group 73.17%(P<0.05).After treatment,the DASS-21 score of the study group was lower than that of the control group(P<0.05).After treatment,the level of CD4^(+)and CD3^(+)in the study group was higher than that in the control group,and the level of CD8^(+)was lower than that in the control group(P<0.05).Conclusion Self-designed Shugan Liqi Jieyu Decoction in treating patients with thyroid carcinoma after operation can significantly relieve clinical symptoms,reduce negative emotions,regulate immune function and improve clinical efficacy.
