摘 要:目的:研究放大内镜技术联合窄带成像技术(ME-NBI)检查早期胃癌及癌前病变的图像质量与诊断效能。方法:选择2021年1月—2023年1月上饶市人民医院收治的43例早期胃癌患者和47例癌前病变患者作为研究对象。患者均行ME-NBI检查及常规内镜检查,评价两种检查方案的图像质量,分析ME-NBI对早期胃癌的诊断效能。结果:ME-NBI检查病灶、胃黏膜表层、微血管图像质量评分均高于常规内镜检查,差异均有统计学意义(P<0.05)。ME-NBI检查诊断早期胃癌的敏感度、准确度均高于常规内镜检查,差异均有统计学意义(P<0.05);但两种检查方案诊断早期胃癌的特异度比较,差异无统计学意义(P>0.05)。ME-NBI图像结果中,早期胃癌患者中存在边界线、微血管不规则、微腺管不规则占比均高于癌前病变患者,差异均有统计学意义(P<0.05)。结论:ME-NBI检查早期胃癌及癌前病变时能获得更加清晰的图像质量,同时对早期胃癌的诊断有更高的敏感度和准确度。Objective:To study the image quality and diagnostic efficiency of magnifying endoscopy combined with narrow band imaging(ME-NBI)in detecting early gastric cancer and precancerous lesions.Method:A total of 43 patients with early gastric cancer and 47 patients with precancerous lesions admitted to Shangrao People's Hospital from January 2021 to January 2023 were selected as the study objects.All patients underwent ME-NBI examination and routine endoscopy,and the image quality of the two examination schemes was evaluated,and analyzed the diagnostic efficacy of ME-NBI in early gastric cancer.Result:The image quality scores of lesions,gastric mucosal surface and microvessels in ME-NBI were higher than those in routine endoscopy,the differences were statistically significant(P<0.05).The sensitivity and accuracy of ME-NBI in the diagnosis of early gastric cancer were higher than those of routine endoscopy,the differences were statistically significant(P<0.05).However,there was no significant difference in the specificity of the two methods in the diagnosis of early gastric cancer(P>0.05).ME-NBI image results showed that the proportion of borderline,microvascular irregularity and microglandular tube irregularity in patients with early gastric cancer were higher than those in patients with precancerous lesions,the differences were statistically significant(P<0.05).Conclusion:ME-NBI can obtain clearer image quality in the detection of early gastric cancer and precancerous lesions,and has higher sensitivity and accuracy in the diagnosis of early gastric cancer.
