摘 要:目的 探讨特异性烯醇化酶(NSE)、血沉(ESR)及C反应蛋白(CRP)在儿童肺炎中的临床应用价值。方法 选取2020年9月~2023年6月在潍坊市人民医院住院治疗的153例肺炎患儿作为实验组,其中肺炎支原体肺炎(MP+)79例,非支原体肺炎(MP-)74例;重度肺炎22例,轻度肺炎131例。选取57例同期门诊健康查体儿童作为对照组。测定各组儿童血清NSE、ESR和CRP水平,采用SPSS25.0软件对实验数据进行分析。结果 (1)实验组血清NSE、ESR、CRP水平均高于对照组(P<0.01)。(2)MP+组与MP-组血清NSE、ESR无差异(P>0.05),但均高于对照组(P<0.05);MP-组CRP水平高于MP+组及对照组,MP+组CRP与对照组无差别(P>0.05)。(3)重度肺炎组血清NSE、CRP高于轻度肺炎组(P<0.05)。(4)诊断肺炎的AUC值,NSE高于CRP和ESR。结论 肺炎患儿NSE水平明显升高,相较于ESR和CRP,NSE对儿童肺炎的诊断以及病情评估具有更重要的临床意义。Objective To explore the clinical application value of specific enolase(NSE),erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)in pediatric pneumonia.Methods One hundred and fifty-three children with pneumonia admitted to Weifang People's Hospital from September 2020 to June 2023 were selected as the experimental group,including 85 cases of Mycoplasma pneumoniae pneumonia(MP+)group,78 cases of non Mycoplasma pneumoniae pneumonia(MP-)group,and 22 cases of severe pneumonia.One hundred and thirty-one cases of common(mild)pneumonia were selected,and 63 children who underwent outpatient health examinations during the same period were selected as the control group.Measure the serum NSE,ESR,and CRP levels of children in each group,and analyze and process the experimental data using SPSS 25.O sofware.Results The serum NSE,ESR,and CRP of the case group were higher than those of the control group(P<0.01).There was no difference in serum NSE and ESR between the MP+group and the MP-group(P>0.05),but both were higher than the control group(P<0.05).The CRP of the MP-group was higher than that of the MP+group and the control group,while there was no difference in CRP between the MP+group and the control group(P>0.05).The serum NSE and CRP in the severe pneumonia group were higher than those in the mild pneumonia group(P<0.05).The AUC value for diagnosing pneumonia is higher in NSE than in CRP and ESR.Conclusion The NSE level in children with pneumonia is significantly elevated,and compared to ESR and CRP,NSE has more important clinical significance in the diagnosis and disease assessment of pediatric pneumonia.
