摘 要:目的探讨不同时段布地奈德雾化吸入对剑突下入路全胸腺切除术后肺功能的影响。方法选取2020年1月-2023年8月在江西省新余市人民医院实施剑突下入路全胸腺切除手术的80例胸腺瘤患者,按随机数字表法分为对照组(n=40)及实验组(n=40)。实验组于术前第3天及术后第1天开始行布地奈德雾化吸入治疗,对照组术前不进行布地奈德吸入治疗,仅术后实施布地奈德雾化吸入治疗。对比两组肺功能[最大通气量(MVV)、第1秒用力呼气量(FEV1)、一氧化碳弥散量(DLCO)、最大呼气流量(PEF)]、血气分析[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2),)、pH值]、住院时间、术后雾化吸入时间、并发症。结果两组术后MVV、FEV1、DLCO、PEF均低于术前,而实验组各项指标水平均较对照组高(P<0.05);两组术后PaO_(2)、pH值均低于术前,PaCO_(2),高于术前,而实验组PaO_(2)、pH值较对照组高,PaCO_(2),较对照组低(P<0.05);实验组住院时间、术后雾化吸入时间均短于对照组(P<0.05);实验组术后并发症发生率较对照组低(P<0.05)。结论术前使用布地奈德雾化吸入可减小剑突下入路全胸腺切除术对患者术后肺功能及血气的影响,并能缩短患者术后雾化吸入时间,减少并发症发生,促进患者病情恢复。Objective To investigate the effects of budesonide inhalation on pulmonary function after subxiphoid approach total thymectomy.Methods A total of 80 thymoma patients who underwent subxiphoid approach total thymotomy in our hospital from January 2020 to August 2023 were selected and divided into control group(n=40)and experimental group(n=40)according to random number table method.The experimental group received budesonide nebulization on the 3rd day before surgery and the Ist day after surgery,while the control group received budesonide nebulization therapy only afer surgery.Pulmonary function[maximum volume of air volume(MVV),forced expiratory volume in the first second(FEV1),carbon monoxide dispersion(DLCO),maximum expiratory flow(PEF)]:blood gas analysis[PaO_(2),PaCO_(2),pH valuel,length of hospital stay,postoperative nebulization time and complications were compared between the two groups.Results The postoperative MVV,FEV1,DLCO,and PEF] in both groups were lower than before surgery,while the experimental group had higher levels of various indicators than the control group(P<0.05).After operation,PaO_(2) and pH values in both groups were lower than before operation,and PaCO_(2) was higher than before operation,while PaO_(2) and pH values in experimental group were higher than control group,PaCO_(2) was lower than control group(P<0.05).The hospital stay and postoperative nebulization time of the experimental group were shorter than those of the control group(P<0.05).The incidence of postoperative complications in the experimental group was lower than that in the control group(P<0.05).Conclusion The use of budesonide nebulization before operation can reduce the effects of subxiphoid approach total thymectomy on lung function and blood gas,shorten the postoperative nebulization time,reduce complications,and promote the recovery of patients.
