摘　　要：目的探讨阿加曲班预防急性缺血性脑卒中危重患者深静脉血栓形成的效果。方法回顾性抽取2023年1月至2023年12月郑州大学第二附属医院收治的急性缺血性脑卒中危重症患者150例,依据治疗用药情况分为观察组(给予常规治疗、氯吡格雷、阿加曲班)和对照组(给予常规治疗、氯吡格雷、低分子肝素钙),每组75例。比较两组深静脉血栓形成率、活化部分凝血活酶时间(APTT)、血浆凝血酶原时间(PT)、治疗总有效率及不良反应发生情况。结果研究组深静脉血栓形成率(4.00%,3/75)、治疗总有效率(96.00%,72/75)、不良反应发生率(1.33%,1/75)与对照组(12.00%,9/75;88.00%,66/75;4.00%,3/75)比较,差异未见统计学意义(P>0.05)。治疗前,两组APTT、PT比较差异未见统计学意义(P>0.05);治疗后,观察组APTT、PT水平低于对照组,P<0.05。结论阿加曲班在预防急性缺血性脑卒中危重症患者围治疗期深静脉血栓形成上效果显著,用药安全性较理想。Objective To investigate the effect of argatroban in preventing deep vein thrombosis in critical patients with acute ischemic stroke.Methods A total of 150 critical patients with acute ischemic stroke admitted to the Second Affiliated Hospital of Zhengzhou University from January 2023 to December 2023 were retrospectively selected,and they were divided into the observation group(treated by conventional treatment,clopidogrel and argatroban)and control group(treated by conventional treatment,clopidogrel and low-molecular-weight heparin calcium)according to the treatment and drug regimens,with 75 cases in each group.The rate of deep vein thrombosis,activated partial thromboplastin time(APTT),plasma prothrombin time(PT),total effective rate of treatment and incidence of adverse reactions were compared between the two groups.Results The rate of deep vein thrombosis(4.00%,3/75),total effective rate(96.00%,72/75)and incidence of adverse reactions(1.33%,1/75)in the study group had no significant difference in comparison with the control group(12.00%,9/75;88.00%,66/75;4.00%,3/75),P>0.05.There was no significant difference in the levels of APTT and PT between the two groups before treatment(P>0.05).The APTT and PT in the observation group were lower than those in the control group after treatment(P<0.05).Conclusions Argatroban has a significant effect in preventing development of deep vein thrombosis in critical patients with acute ischemic stroke during the perioperative period,and the medication safety is relatively ideal.
