高频超声对乳腺癌患者内乳淋巴结转移的诊断价值

摘　　要：目的探讨高频超声对乳腺癌患者内乳淋巴结(IMLN)转移的诊断价值。方法回顾性分析2018年7月至2020年9月我院收治的102例疑似发生IMLN转移的乳腺癌患者的临床资料,根据手术或穿刺病理学检查结果将其分为转移组(60例,发生IMLN转移)和未转移组(42例,未发生IMLN转移)。所有患者术前进行高频超声检查,分析两组患者的超声图像特点,评估高频超声对乳腺癌患者IMLN转移的诊断价值。结果转移组IMLN长径、厚径均明显高于未转移组(P<0.05);两组的IMLN长径/厚径比值以及血流信号超声表现比较无统计学差异(P>0.05)。ROC曲线结果显示,IMLN长径、厚径联合诊断乳腺癌患者发生IMLN转移的AUC为0.870,高于单独指标诊断。结论高频超声下IMLN长径及厚径对乳腺癌IMLN转移具有较高的诊断价值,值得临床借鉴参考。Objective To explore the diagnostic value of high-frequency ultrasound for internal mammary lymph node(IMLN)metastasis of patients with breast cancer.Methods The clinical data of 102 breast cancer patients with suspected IMLN metastasis admitted to our hospital from July 2018 to September 2020 were retrospectively analyzed.According to the results of surgery or puncture pathology,the patients were divided into metastasis group(60 cases,with IMLN metastasis)and non-metastasis group(42 cases,without IMLN metastasis).All patients were given high-frequency ultrasound before surgery.The features of ultrasound images of the two groups were analyzed,and the diagnostic value of high-frequency ultrasound for IMLN metastasis of breast cancer patients was evaluated.Results The IMLN long diameter and thick diameter of the metastasis group were significantly higher than those of the non-metastasis group(P<0.05);No statistical difference was found in the IMLN long diameter/thick diameter ratio and the ultrasonic manifestations of IMLN blood flow signals between the two groups(P>0.05).ROC curve results showed that the AUC of combined diagnosis of IMLN long diameter and thick diameter for IMLN metastasis in breast cancer patients was 0.870,higher than that of single index diagnosis.Conclusions IMLN long diameter and thick diameter under high-frequency ultrasound has high diagnostic value for IMLN metastasis in breast cancer patients,which is worthy ofclinical reference.
