左右颈7交叉移位术对脑部疾病偏瘫患者心脏自主神经功能的影响

摘　　要：目的:探究左右颈7交叉移位术对脑部疾病偏瘫患者心脏自主神经功能的影响,为其安全性评估及后续临床应用提供参考。方法:选取2021年9月—2023年1月在上海市静安区中心医院行左右颈7交叉移位术的70例脑部疾病偏瘫患者,比较其术前、术后3 d、术后1个月的心率和血压情况。同时比较患者术前、术后1个月的动态血压、肾素-血管紧张素-醛固酮系统指标(renin-angiotensin-aldosterone system,RAAS)、超声心动图指标及心率变异性。结果:术前、术后3 d、术后1个月,患者心率、收缩压及舒张压比较,差异均有统计学意义(F值分别为63.898、68.620、131.106,P<0.05),且均随时间发展呈先升高后降低的趋势。术后1个月,患者动态血压、RAAS指标、超声心动图指标、心率变异性与术前比较,差异均无统计学意义(P>0.05)。结论:左右颈7交叉移位术可能引起患者心率、血压波动,但对患者RAAS、超声心动图指标及心率变异性无明显影响。OBJECTIVE To investigate the influence of contralateral seventh cervical nerve transfer on cardiac autonomic nerve function of patients with brain disease hemiplegia.METHODS From September 2021 to January 2023,70 patients with brain disease hemiplegia who underwent left and right neck 7-cross transposition in Shanghai Jing’an District Central Hospital were selected.Heart rate(HR)and blood pressure were compared before operation,3 days and 1 month after operation.Ambulatory blood pressure,renin-angiotensin-aldosterone system(RAAS)index,echocardiography index and heart rate variability were compared before and 1 month after operation.RESULTS The HR,systolic blood pressure(SBP)and diastolic blood pressure(DBP)of patients before surgery,3 days and 1 month after surgery showed a trend of firstly increasing and then decreasing with the development of time,and the 3 days after was apparently higher compared before surgery and 1 month after surgery(F=63.898,68.620,131.106,P<0.05).There was no significant difference in ambulatory blood pressure,RAAS index,echocardiography index and heart rate variability between 1 month after operation and before operation(P>0.05).CONCLUSION Contralateral seventh cervical nerve transfer may cause the fluctuation of HR and blood pressure,but it had no significant effect on RAAS,heart rate variability and echocardiogram indexes.
