摘　　要：目的:探讨脑脊液中降钙素原(PCT)及白细胞介素-6(IL-6)的表达对开颅术后无菌性脑膜炎的诊断价值。方法:回顾性分析2019年2月—2021年2月于泰州市第四人民医院神经外科行开颅手术的126例患者临床资料,根据脑脊液白细胞计数(WBC)>100×10^(6)/L且葡萄糖(GLU)<2.5 mmol/L将手术患者分成化脓性脑膜炎组(30例)和无菌性脑膜炎组(96例)。对两组患者的一般资料进行分析,采用单因素和多因素Logistic回归分析脑脊液化脓性、无菌性脑膜炎的相关危险因素,应用受试者工作特征(ROC)曲线下面积(AUC)分析WBC、GLU、PCT、IL-6在鉴别化脓性和无菌性脑膜炎中的诊断效能。结果:单因素和多因素Logistic回归分析显示,化脓性和无菌性脑膜炎患者脑脊液PCT、IL-6、WBC、GLU水平比较,差异有统计学意义(P<0.05)。脑脊液PCT、IL-6、WBC、GLU是鉴别化脓性脑炎、无菌性脑膜炎的重要指标。结论:脑脊液中PCT及IL-6的表达对开颅术后无菌性脑膜炎的诊断及治疗有实际临床指导价值。Objective:To investigate the value of procalcitonin(PCT)and interleukin-6(IL-6)expression in cerebrospinal fluid in the diagnosis of aseptic meningitis after craniotomy.Methods:The clinical data of one hundred and twenty-six patients who underwent craniotomy in the Department of Neurosurgery of Taizhou Fourth People's Hospital from February 2019 to February 2021 were retrospectively analyzed.According to cerebrospinal fluid WBC>100×10^(6)/L and glucose(GLU)<2.5 mmol/L,the patients were divided into suppurative meningitis group(30 cases)and aseptic meningitis group(96 cases).The general data of the two groups were analyzed,and the risk factors of cerebrospinal fluid suppurative and aseptic meningitis were analyzed by univariate and multivariate Logistic regression.The area under receiver operating characteristic(ROC)curve(AUC)was used to analyze the diagnostic efficacy of WBC,GLU,PCT and IL-6 in differentiating suppurative and aseptic meningitis.Results:Univariate and multivariate Logistic regression analysis showed that there were statistically significant differences in PCT,IL-6,WBC and GLU levels in cerebrospinal fluid between suppurative and aseptic meningitis patients(P<0.05);cerebrospinal fluid PCT,IL-6,WBC and GLU are important indexes for distinguishing suppurative encephalitis and aseptic meningitis.Conclusion:The expression of PCT and IL-6 in cerebrospinal fluid has practical clinical guiding value for the diagnosis and treatment of aseptic meningitis after craniotomy.
