综合护理在脑出血置管吸引手术患者中的应用

摘　　要：目的分析接受脑出血置管吸引手术的患者应用综合护理对促进其预后改善所发挥的作用。方法将132例自2022年1月至2023年10月在厦门大学附属第一医院杏林分院接受置管吸引术的脑出血患者以随机数字表法分为两组,对照组(66例)和观察组(66例)患者围手术期分别应用常规护理和综合护理,比较两组睡眠障碍与神经功能缺损改善效果、上、下肢肢体功能、生活质量、并发症和满意度。结果干预1、2周后及干预1、3个月后观察组匹兹堡睡眠质量指数量表(PSQI)评分和美国国立卫生院卒中量表(NIHSS)评分均较对照组低(均P<0.05);干预1、3个月后观察组上肢、下肢Fugl-Meyer运动功能评估量表(FMA)评分及总评分均较对照组更高(均P<0.05);干预后观察组简明健康调查量表(SF-36)活力、生理功能、社会功能、生理职能、情感职能、精神健康、躯体疼痛和总体健康等各项得分以及患者对护理的满意度均高于对照组(均P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论接受脑出血置管吸引手术的患者应用综合护理既可促进其睡眠质量改善,还有助于促进神经功能、运动功能及生活质量改善,而且可控制并发症,提高患者护理满意度。Objective To analyze the role of comprehensive nursing in promoting the improvement of prognosis in patients undergoing catheterization and suction surgery for cerebral hemorrhage.Methods A total of 132 patients with cerebral hemorrhage who underwent catheterization and suction surgery at the Xinglin Branch of the First Affiliated Hospital of Xiamen University from January 2022 to October 2023 were divided into two groups using a random number table method.The control group(66 cases)and the observation group(66 cases)received routine care and comprehensive care during the perioperative period respectively.The Pittsburgh Sleep Quality Index(PSQI),National Institutes of Health Stroke Scale(NIHSS),Fugl-Meyer assessment(FMA),The MOS item short from health survey(SF-36),complications,and nursing satisfaction were compared.Results The Pittsburgh Sleep Quality Index(PSQI)scores and National Institutes of Health Stroke Scale(NIHSS)scores were lower in the observation group than in the control group after 1 and 2 weeks of the intervention and after 1 and 3 months of the intervention(both P<0.05);the upper and lower extremity Fugl-Meyer assessment(FMA)scores and total scores were higher in the observation group than in the control group after 1 and 3 months of intervention(all P<0.05);The MOS item short from health survey(SF-36)for vitality,physical functioning,social functioning,physiological functioning,emotional functioning,mental health,somatic pain,and general health,as well as patient satisfaction with care,were higher in the observation group than in the control group(all P<0.05);The complication rate in the observation group was lower than that in the control group(P<0.05).Conclusions The application of comprehensive nursing in patients undergoing intracerebral hemorrhage catheterization and suction surgery can not only promote the improvement of their sleep quality,but also contribute to the improvement of neurological function,motor function,and quality of life.It can also control complications and improve patien
