参芪益气养血汤治疗胃癌化疗患者癌因性疲乏临床研究

摘　　要：目的:探讨参芪益气养血汤治疗胃癌化疗患者癌因性疲乏(CRF)的效果。方法:选取在本院就诊的94例胃癌化疗CRF患者为研究对象,随机分为对照组和治疗组,各47例。对照组予以常规化疗方案治疗,治疗组在对照组的基础上采用参芪益气养血汤治疗。比较两组患者治疗前后中医症候积分、Piper疲乏调查量表(PFS)评分、免疫功能指标[自然杀伤(NK)细胞、CD_(4)^(+)细胞、CD_(8)^(+)细胞]、炎症指标[C反应蛋白(CPR)、白介素-2(IL-4)和白介素-6(IL-6)]以及治疗期间不良反应情况。结果:治疗后,两组中医症候积分低于治疗前,且治疗组低于对照组(P<0.05);两组PFS评分低于治疗前,且治疗组低于对照组(P<0.05);两组NK和CD_(4)^(+)水平低于治疗前,CD_(8)^(+)水平高于治疗前,且治疗组NK和CD_(4)^(+)水平高于对照组,CD_(8)^(+)水平低于对照组(P<0.05);两组CPR、IL-4及IL-6水平低于治疗前,且治疗组CPR、IL-4及IL-6水平低于对照组(P<0.05);治疗期间,两组安全性情况无明显差异(P>0.05)。结论:参芪益气养血汤治疗胃癌化疗CRF患者,可以缓解中医症状和疲乏程度,增强免疫功能,抑制炎症反应,且安全性好。Objective To explore the curative effect of Shenqi Yiqi Yangxue Decoction on cancer-related fatigue(CRF)in patients after gastric cancer chemotherapy.Methods A total of 94patients with CRF after gastric cancer chemotherapy in the hospital were enrolled as the research objects.They were randomly divided into control group and treatment group,47cases in each group.The control group was given routine chemotherapy,while treatment group was additionally treated with Shenqi Yiqi Yangxue Decoction.The scores of TCM symptoms and Piper Fatigue Scale(PFS),immune function indexes[natural killer(NK)cells,CD_(4)^(+)cells,CD_(8)^(+)cells]and inflammatory indexes[C-reactive protein(CPR),interleukin-4(IL-4),interleukin-6(IL-6)]before and after treatment,and adverse reactions during treatment were compared between the two groups.Results After treatment,scores of TCM symptoms in both groups were decreased,which were lower in treatment group than control group(P<0.05).After treatment,PFS score in both groups was decreased,which was lower in treatment group than control group(P<0.05).After treatment,NK and CD_(4)^(+)levels were decreased,while CD_(8)^(+)level was increased in both groups.NK and CD_(4)^(+)levels in treatment group were higher than those in control group,while CD_(8)^(+)level was lower than that in control group(P<0.05).After treatment,levels of CPR,IL-4and IL-6in both groups were decreased,which were lower in treatment group than control group(P<0.05).During treatment,there was no significant difference in the safety between the two groups(P>0.05).Conclusion Shenqi Yiqi Yangxue Decoction can relieve TCM symptoms and fatigue,enhance immune function and inhibit inflammatory response in patients with CRF after gastric cancer chemotherapy,with good safety.
