增强CT与MRI多期增强扫描对小肝癌诊断价值比较

摘　　要：目的:观察增强CT、MRI多期增强扫描对小肝癌(SHCC)的诊断价值。方法:选择在广东省人民医院就诊且经病理检查确诊的70例SHCC患者,均行MRI、CT检查,评价二者检出率。结果:①病理检查显示70例SHCC患者共80个病灶,MRI的总检出率为96.25%,显著高于CT的81.25%(P<0.05);②MRI检查中T1W1检出55例(71.43%)、T2W1检出59例(76.62%)、动脉期检出68例(88.31%)、门静脉期检出44例(57.14%)、平衡期检出58例(75.32%),增强扫描中动脉期检出率相对较高;③CT检查中平扫检出38例(58.46%)、动脉期检出57例(87.69%)、门静脉期检出40例(61.54%)、平衡期检出46例(70.77%),增强扫描中动脉期检出率相对较高。结论:较之CT,MRI在诊断SHCC时检出率高,能够提供可靠的早期诊断依据。Objective:To observe the diagnostic value of multi-phase contrast-enhanced computed tomography(CT)and magnetic resonance imaging(MRI)in small hepatocellular carcinoma(SHCC).Methods:A total of 70 SHCC patients who were admitted to Guangdong Provincial People's Hospital from April 2019 to June 2021 and confirmed through pathological examination were selected,all of whom underwent MRI and CT to evaluate their detection rates.Results:Pathological examination showed a total of 80 lesions in 70 SHCC patients,with a total detection rate of 96.25%on MRI,significantly higher than 81.25%on CT,P<0.05.In MRI examination,55 cases(71.43%)of T1W1 were detected,59 cases(76.62%)of T2W1 were detected,68 cases(88.31%)were detected in arterial phase,44 cases(57.14%)were detected in portal phase,and 58 cases(75.32%)were detected in balance phase.The detection rate of the arterial phase in contrast-enhanced scanning was relatively high.In CT examination,38 cases(58.46%)were detected in plain scan,57 cases(87.69%)in the arterial phase,40 cases(61.54%)in portal phase,and 46 cases(70.77%)in the equilibrium phase.The detection rate of the arterial phase in the enhanced scan was relatively high.Conclusion:Compared to CT,MRI has a higher detection rate in diagnosing SHCC and can provide a reliable early diagnostic basis,which is worth promoting.
