摘　　要：目的调查国内三级甲等医院骨科护理人员实施体位管理现状,针对骨科体位管理存在问题提出对策。方法2023年8—10月采用自行设计的调查问卷,对我国13个省份27所三甲医院968名骨科护士实施体位管理的现状进行问卷调查。结果27所医院中59%未建立专门的体位管理指引,30%缺乏完善的体位管理指引,仅26%针对体位管理进行相关培训;体位管理方法认知得分为(3.5±0.6)分,仅5.8%的护士掌握体位管理原则,上肢良肢位摆放方法得分为0;76.3%骨科护士认为,体位管理相关知识掌握不够是影响其执行体位管理的主要因素;在培训需求方面,骨科护士对制作体位健康宣传册(78.9%)和临床操作示范(76.3%)需求最为迫切。结论国内三甲医院骨科护士的体位管理现况不容乐观,护理管理者重视程度不够,护士认知率偏低。急需制定健全的骨科患者体位管理制度,针对需求加强骨科护士专项培训,积极研发和应用体位护理用具,以更好地实施系统性体位管理。Objective To investigate the current status of position management for orthopaedic nurses in tertiary first-class hospitals in China,and to provide the countermeasures focus on the existing problems in orthopaedic position management.Methods A survey of current status of position management was performed from August 2023 to October 2023 on 968 orthopaedic nurses from 13 provinces,27 domestic tertiary first-class hospitals according to a self-designed questionnaire.Results In 27 hospitals,there were 59%hospitals with lack of specialized position management guidance,30%hospitals with incomplete position management guidance,only 26%hospitals carried out the training of position management.Average cognitive score of posture management was(3.5±0.6)point;only 5.8%could master position management principles,and the score of mastering the method of normal upper limb positioning was 0.A total of 76.3%of orthopaedic nurses thought that inadequate knowledge of position management was the major factor affecting orthopaedic nurses'implementation of posture management.As for training requirements,the most urgent needs of orthopaedic nurses were to make health pamphlets of posture management(78.9%)and to demonstrate standardized clinical operations(76.3%).Conclusions The current situation of position management for orthopaedic nurses in tertiary first-class hospital in our country was not optimistic,attention from nursing managers is not enough,and the mastery of position management knowledge is relative low.It is necessary to formulate well-established position management system for orthopaedic patients, enhance special training focus on orthopaedic nurses' needs, and develop and apply position nursing appliances actively, so as to help nurses to implement systematic position management better.
