摘 要:目的探讨宫腔镜息肉电切术后miR-152及血管内皮生长因子(VEGF)水平与复发的相关性。方法回顾性分析2020年8月至2022年12月到本院行宫腔镜息肉电切术的130例患者作为纳入对象,根据术后1年内息肉是否复发,分为复发组(n=26)及未复发组(n=104)。记录并比较两组患者基线资料、miR-152、VEGF、性激素及炎症反应程度,采用Spearman法进行相关性分析,并采用Logistic多因素分析影响复发的相关因素。结果复发组miR-152水平低于未复发组,VEGF及白细胞介素-6(IL-6)水平高于未复发组(P<0.05)。miR-152与复发情况呈负相关(r=-0.381,P<0.05),VEGF与复发情况呈正相关(r=0.388,P<0.05)。复发组促卵泡生成素(FSH)、促黄体生成素(LH)及雌二醇(E2)水平均高于未复发组(P<0.05)。Logistic分析结果得出,miR-152水平偏小,息肉直径、VEGF、IL-6及E2水平偏高为影响患者术后复发的相关因素。结论miR-152及VEGF水平与宫腔镜息肉电切术后复发具有密切关系,并且发现息肉直径、IL-6及E2水平偏高同样将增加患者术后复发风险。Objective To investigate the correlation between miR-152 and vascular endothelial growth factor(VEGF)levels and recurrence after hysteroscopic polypectomy.Methods 130 patients who came to our hospital for hysteroscopic polypectomy from August 2020 to December 2022 were retrospectively analyzed as the inclusion subjects,and were divided into the recurrence group(n=26)and the non-recurrence group(n=104)according to whether the polyps recurred within 1 year after the operation.Baseline data,miR-152,VEGF,sex hormones and the degree of inflammatory response were recorded and compared between the two groups,and correlation analysis was performed using Spearman's method,and logistic multifactorial analysis was used to analyze the relevant factors affecting recurrence.Results The level of miR-152 was lower in the recurrence group than that in the non-recurrence group,and the levels of VEGF and interleukin-6(IL-6)were higher than that in the non-recurrence group(P<0.05).miR-152 was negatively correlated with recurrence(r=-0.381,P<0.05)and VEGF was positively correlated with recurrence(r=0.388,P<0.05).The levels of follicle stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E2)were higher in the relapse group than those in the non-relapse group(P<0.05).Logistic analysis results concluded that small miR-152 levels,high polyp diameter,VEGF,IL-6 and E2 levels were the relevant factors affecting the patients'postoperative recurrence.Conclusion The levels of miR-152 and VEGF were strongly associated with recurrence after hysteroscopic polypectomy,and it was found that high polyp diameter,IL-6 and E2 levels also increased the risk of postoperative recurrence in patients.
