摘 要:目的研究头颈部肿瘤放疗患者发生放射性口腔黏膜炎的相关影响因素。方法对2020年10月至2023年10月杭州市肿瘤医院收治的130例头颈部肿瘤放疗患者的临床资料进行回顾性分析,以患者是否发生放射性口腔黏膜炎将其分为研究组(发生,n=110)和对照组(未发生,n=20),应用单因素χ^(2)检验和多因素Logistic回归分析法分析患者发生放射性口腔黏膜炎的影响因素。结果单因素分析结果显示,放射性口腔黏膜炎发生与吸烟史、诱导化疗、口腔卫生状况、应用口腔黏膜保护剂情况有关(P<0.05)。多因素Logistic回归分析结果显示,放射性口腔黏膜炎发生与诱导化疗、未使用黏膜保护剂、吸烟、口腔卫生差均有关,其是导致放射性口腔黏膜炎发生的独立危险因素(P<0.05)。结论头颈部肿瘤放疗患者发生放射性口腔黏膜炎的几率较高,其中诱导化疗、吸烟、未使用黏膜保护剂、口腔卫生差是影响其发生放射性口腔黏膜炎的独立危险因素,因此在临床工作中可针对性预防以降低放射性口腔黏膜炎发生风险。Objective To study on the related influencing factors of radiation-induced oral mucosal inflammation in patients with head and neck tumor radiotherapy.Methods A retrospective analysis was conducted on the clinical data of 130 patients with head and neck tumor radiotherapy admitted to our hospital from October 2020 to October 2023.The patients were divided into the study group(occurrence,n=110)and the control group(no occurrence,n=20)based on the incidence of radiation-induced oral mucositis.Single factor χ^(2) inspection and multivariate logistic regression analysis were applied to analyze the relevant influencing factors of radiation induced oral mucosal inflammation in patients.Results The results of univariate analysis showed that the incidence of radiation induced oral mucositis was related to history of smoking,induction chemotherapy,oral hygiene status,and use of oral mucosal protectants(P<0.05).The results of multivariate logistic regression analysis showed that the occurrence of radiation-induced oral mucositis was related to induced chemotherapy,no use of mucosal protectants,smoking,and poor oral hygiene,it was an independent risk factor for radiation-induced oral mucositis(P<0.05).Conclusion Patients with head and neck tumors undergoing radiotherapy have a higher risk of developing radiation-induced oral mucosal inflammation.The radioactive oral mucositis is associated with induced chemotherapy,no use of mucosal protective agents,smoking,and poor oral hygiene,which are independent risk factors,therefore,targeted prevention can be made in clinical work to reduce the risk of radioactive oral mucositis.
