摘 要:目的系统评价乳酸菌治疗宫颈人乳头瘤病毒(HPV)感染的疗效与安全性。方法使用检索词“乳酸菌、HPV感染”等,检索PubMed、The CochraneLibrary、Web of Science、WanFang Data、CBM、CNKI和VIP数据库,检索时间为各大数据库建库至2023年9月17日。2位研究人员分别对文献进行筛选,抽取数据,并对偏倚风险进行评估。结果共获得1085篇文章,最终纳入46篇文献。Meta分析结果显示:乳酸菌组治疗宫颈HPV感染的有效率为对照组的1.24倍[95%CI(1.21,1.27),P<0.05]、转阴率为对照组的1.37倍[95%CI(1.30,1.44),P<0.05],Nugent评分、阴道pH值、白介素(IL)-4与对照组相比降幅更大(P<0.05),IL-12、干扰素-(IFN-)与对照组相比升幅更大(P<0.05),但不良反应发生率与对照组比较差异无统计学意义(P>0.05)。结论乳酸菌对于治疗宫颈HPV感染疗效显著,但不良反应尚不明确。Objective To evaluate the efficacy and safety of lactic acid bacteria in the treatment of cervical human papillomavirus(HPV)infection,Methods The search term"lactic acid bacteria,HPV infection"was used to search PubMed,The Cochrane Library,Web of Science,WanFang Data,CBM,CNKI and VIP databases,and the search time was from the establishment of all major databases to September 17,2023.The two researchers separately screened the literature,extracted data,and assessed the risk of bias.Results A total of 1085 articles were obtained,and 46 articles were included.The results of meta-analysis showed:The effective rate of the lactobacillus group in the treatment of cervical HPV infection was 1.24 times that of the control group[95%CI(1.21,1.27),P<0.05],and the negative conversion rate was 1.37 times that of the control group[95%CI(1.30,1.44),P<0.05].Nugent score,vaginal pH and interleukin(IL)-4 decreased significantly compared with the control group(P<0.05),and IL-12 and interferon-y(IFN-v)increased significantly compared with the control group(P<0.05),but the incidence of adverse reactions was not statistically significant compared with the control group(P>0.05).Conclusion Lactic acid bacteria is effective in the treatment of cervical HPV infection,but the side effects are notclear.
