摘　　要：目的探究慢性精神分裂症患者骨密度(bone mineral density,BMD)的变化情况及其影响因素。方法将贵州铜仁市第三人民医院住院治疗的135名慢性精神分裂症患者按照骨密度情况分为骨量正常组、骨量减少组和骨质疏松组,检测甘油三酯、总胆固醇、低密度脂蛋白、高密度脂蛋白、空腹血糖和糖化血红蛋白水平。比较三组一般人口学资料及临床资料,分析影响骨密度的主要因素。结果慢性精神分裂症患者骨量减少的发生率为19.3%(26例),骨质疏松的发生率为11.1%(15例)。与骨量正常组比较,骨质疏松组的年龄显著增高,体重指数显著减少,阴性症状量表总分更高。多元逐步回归分析显示年龄、阴性症状量表总分与BMD呈负相关,糖化血红蛋白与BMD呈正相关。结论年龄较大,糖化血红蛋白水平较低,阴性症状更重是慢性精神分裂症患者骨密度降低的重要因素。Objective To investigate the alterations in bone mineral density(BMD)of patients with chronic schizophrenia and its influencing factors.Methods A total of 135 patients hospitalized at the Third People's Hospital of Tongren City,Guizhou Province were divided into normal bone mass group,decreased bone mass group,and osteoporosis group based on their BMD.Levels of triglyceride,total cholesterol,low-density lipoprotein,high-density lipoprotein,fasting glucose,and glycated hemoglobin were detected.General demographic data and clinical information of the three groups were compared to examine the key determinants affecting BMD.Results The incidence of decreased bone mass was 19.3%(26 cases),while that of osteoporosis was 11.1%(15 cases).Compared with the normal bone mass group,patients in the osteoporosis group had a significantly higher age,lower BMI,and a greater SANS total score.Multivariate stepwise regression analysis revealed a negative correlation between age,SANS total score,and BMD,and a positive correlation between glycated hemoglobin and BMD.Conclusion Older age,lower glycated hemoglobin level,and more severe negative symptoms are important predictive variables for reduced BMD in patients with chronic schizophrenia.
