摘 要:目的探讨抑郁症患者的中缝核(MidBrain Raphe,MBR)回声与脑血管反应性(Cerebral Vascular Reactivity,CVR)的相关性。方法选择2023年1月~2023年9月我院精神科门诊就诊的60例抑郁症患者为研究组,另选同期健康体检的70名正常人为对照组。记录并比较两组患者的年龄、性别及文化程度、屏气指数(Breath-Holding Index,BHI)、MBR回声异常的阳性率的差异,然后分析汉密尔顿抑郁量表(Hamilton Depression Rating Scale,HAMD)量表评分与BHI及MBR回声异常之间的相关性。结果两组基本临床资料未见明显差异(P>0.05);研究组的BHI(0.58±0.09对0.84±0.10,t=-14.862,P=0.000)明显高于对照组(P<0.05);研究组MBR回声减低的阳性率(87.5%对28.6%,χ^(2)=27.857,P=0.000)明显高于对照组(P<0.05),余指标差异比较无统计学意义(P>0.05);研究组的MBR回声减低随HAMD量表评分升高而降低,呈负相关关系(r=-0.325;P=0.011);而BHI与HAMD量表并无线性相关(r=-0.221;P>0.05)。结论MBR回声减低与HAMD量表存在负相关关系,对于无法完全配合HAMD量表评估的患者,可使用经颅超声检测MBR回声可以作为诊断抑郁症的辅助方法。Objective To investigate the correlation between midbrain raphe(MBR)echo and cerebral vascular reactivity(CVR)in patients with depression.Methods Sixty patients diagnosed with depression who visited the psychiatry outpatient clinic of our hospital from January 2023 to September 2023 were selected as the study group,and 70 healthy individuals undergoing physical examination in the same period were selected as the control group.Record and compare the age,gender,educational levels,Breath-Holding Index(BHI),and positivity rates of MBR echo abnormalities between two patient groups.Subsequently,analyze the correlation between Hamilton Depression Rating Scale(HAMD)scores and both BHI and MBR echo abnormalities.The correlation between Hamilton Depression Rating Scale(HAMD)scores and BHI and abnormal MBR echo was then analyzed.Results There was no significant difference in the basic clinical data between the two groups(P>0.05).The BHI of the study group(0.58±0.09 vs.0.84±0.10,t=-14.862,P=0.000)was significantly higher than that of the control group(P<0.05).The positive rate of decreased MBR echo in the study group(87.5%vs.28.6%,χ^(2)=27.857,P=0.000)was significantly higher than that in the control group(P<0.05),with no statistically significant difference in other indices(P>0.05).The decrease in MBR echo in the study group was negatively correlated with the increase in HAMD scores(r=-0.325,P=0.011),while there was no linear correlation between BHI and HAMD scores(r=-0.221,P>0.05).Conclusion A decrease in MBR(midbrain echo)is negatively correlated with HAMD(Hamilton Depression Rating Scale)scores.For patients unable to fully cooperate with the HAMD scale assessment,using transcranial ultrasound to detect MBR echo can serve as an auxiliary method for diagnosing depression.
