摘　　要：目的探讨阿立哌唑联合文拉法辛治疗重度抑郁症患者的效果及对其血清炎性因子、神经营养因子影响。方法本研究选择2020年1月~2022年12月124例重度抑郁症患者,数字表法随机分为两组。对照组采用文拉法辛治疗,观察组采用阿立哌唑联合文拉法辛治疗,均治疗8周。比较两组治疗前后汉密尔顿抑郁量表17项(HAMD-17)、血清多巴胺(DA)、5-羟色胺(5-HT)水平,检测治疗前后脑源性神经营养因子(BDNF)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平。结果两组治疗后HAMD-17评分均显著降低,血清5-HT、DA水平显著升高,且治疗后观察组HAMD-17评分低于对照组,5-HT、DA水平高于对照组,(P<0.05);两组治疗后血清IL-6、TNF-α水平均显著降低,BDNF水平升高,且治疗后观察组IL-6、TNF-α水平低于对照组,BDNF水平高于对照组(P<0.05)。结论阿立哌唑联合文拉法辛治疗重度抑郁症能有效减轻抑郁情绪,提升5-HT、DA水平,能够调节神经营养因子及降低炎症反应。Objective To explore the effect of aripiprazole combined with venlafaxine on major depression patients and their serum inflammatory factors and neurotrophic factors.Methods 124 major depression patients were randomly divided into two groups using the number table method.The control group was treated with venlafaxine,and the observation group was treated with aripiprazole combined with venlafaxine for 8 weeks.The levels of 17 items of Hamilton Depression Scale(HAMD-17),serum dopamine(DA)and 5-hydroxytryptamine(5-HT)before and after treatment in the two groups were compared.Brain derived neurotrophic factor(BDNF),interleukin 6(IL 6),tumor necrosis factorα(TNF-α)level were detected before and after treatment.Results After treatment,the HAMD-17 score of the two groups decreased significantly,and the serum levels of 5-HT and DA increased significantly.After treatment,the HAMD-17 score of the observation group was lower than that of the control group,and the levels of 5-HT and DA were higher than that of the control group(P<0.05).Serum IL-6 and TNF-αafter treatment in both groups levels of BDNF were significantly decreased,and the level of BDNF was increased.After treatment,The IL-6,and TNF-αlevel of observation group were lower than that of control group,and the level of BDNF was higher than that of the control group(P<0.05).Conclusion The treatment of severe depression with aripiprazole and venlafaxine can effectively reduce depression,improve the levels of 5-HT and DA,and can reduce the inflammatory reaction and neurotrophic factors.
