手术及放疗后复发的腹膜后去分化脂肪肉瘤一例并文献复习

摘　　要：目的分析腹膜后去分化脂肪肉瘤的诊疗经验,以期为临床提供参考。方法回顾性分析我院一例去分化脂肪肉瘤患者的诊断和治疗经过以及随访情况。结果患者因腹痛就诊,行CT及MRI检查提示腰腹部间充质肉瘤后行手术切除,术后经病理及荧光原位杂交技术诊断为去分化脂肪肉瘤,后随访发现复发并再次手术,之后随访再次发现复发,患者遂接受放疗,随访发现病灶无增大及转移。结论去分化脂肪肉瘤罕见,易复发,预后差,诊断主要依靠MRI、CT、病理、荧光原位杂交技术,初发和复发首选手术治疗,反复复发放疗也可获益。Objective The diagnosis and treatment experience of retroperitoneal dedifferentiated liposarcoma was analyzed to provide clinical reference.Methods The clinical data of a patient with dedifferentiated liposarcoma admitted to our hospital were analyzed retrospectively.Results The primary symptom was abdominal pain.She was diagnosed as abdominal mesenchymal sarcoma by CT and MRI.Surgical resection of the sarcoma was conducted.After surgery,the patient was diagnosed as dedifferentiated liposarcoma by pathology and fluorescence in situ hybridization.Recurrence was found later and the patient went through a surgery.After the recurrence was found again,the patient underwent radiotherapy and the tumor didn’t get large or metastasise.Conclusion Dedifferentiated liposarcoma is rare and easy to relapse.It also has a poor prognosis.The diagnosis mainly depends on MRI,CT,pathology and fluorescence in situ hybridization.Surgery can be the first selection for first onset and recurrence,but for the repeated recurrentce,radiotherapy can also benefit.
