Forsus矫治器治疗安氏Ⅱ类错[牙合]患者上气道变化的CBCT研究

摘　　要：目的探讨Forsus矫治器治疗生长发育期安氏Ⅱ类错[牙合]畸形患者前后上气道的变化。方法选取安氏Ⅱ类下颌后缩生长期患者44例,实验组采用Forsus功能矫治器+直丝弓矫治器,对照组未进行功能矫治。在治疗前后拍摄锥体束计算机断层扫描(CBCT),影像采用Invivo 5软件和Dolphin软件进行测量,数据采用SPSS 22.0统计软件进行统计学分析。结果实验组治疗后上气道总体积、腭咽体积、口咽体积增加,软腭尖平面处的截面积Ssp、口咽段气道的最小截面积S-min、硬腭平面处的气道矢横径之比AP/L_(HP)增加,软腭尖平面处的气道矢横径之比AP/L_(SP)减小,实验组治疗前后有统计学差异(P<0.05),且组间有统计学差异(P<0.05)。结论利用Forsus矫治器可以改善青少年安氏Ⅱ类下颌后缩患者的上气道容积与通气功能,其对于腭咽部气道的影响最为突出。Objective To explore the changes of the upper airway before and after the treatment of patients with Class II malocclusion during the growth and development period with Forsus appliance.Methods A total of 44 patients with Class II malocclusion mandibular retraction were enrolled.The experimental group was treated with Forsus appliance and straight wire appliance,and the control group was not treated with functional correction.Cone beam computed tomography(CBCT)was taken anteriorly and posteriorly,and the data were measured by Invivo 5 software and Dolphin software.The data were statistically analyzed by SPSS22.0 statistical software.Results After treatment,the total upper airway volume,velopharyngeal volume and oropharyngeal volume increased,the crosssectional area at the soft palate apical plane,the minimum cross-sectional area of the oropharyngeal airway,the sagittal transverse diameter ratio at the hard palate plane increased,and the sagittal transverse diameter ratio at the soft palatine apical plane decreased,and there were statistical differences before and after treatment(P<0.05),and there were statistical differences between groups(P<0.05).Conclusion The use of Forsus appliance in the treatment of adolescents with ClassⅡmandibular retraction can increase the volume of the upper airway and improve the ventilation function of the patients,and its impact on the velopharyngeal airway is the most prominent.
