摘 要:目的 探讨首发抑郁症患者静息态功能磁共振成像(rs-fMRI)多网络脑功能改变情况及与临床症状的相关性。方法 收集22例首发抑郁症患者(研究组)和18例健康对照(对照组),分别对两组行rs-fMRI扫描,以后扣带回(PCC)作为默认网络(DMN)的种子点,前岛叶(INS)作为突显网络(SN)种子点,背外侧前额叶(DLPFC)作为认知控制网络(CCN)种子点,采用基于种子点的功能连接(FC)分析方法观察两组在DMN、SN和CCN的差异,并完成首发抑郁症患者24项汉密尔顿抑郁量表(HAMD-24)和汉密尔顿焦虑量表(HAMA)评定,分析差异脑区与临床症状的相关性。结果 在DMN中,研究组左侧PCC与右侧枕下回的FC较对照组降低,右侧PCC与左侧角回的FC较对照组降低。在SN中,研究组左侧前岛叶与左侧颞下回、右侧枕上回的FC较对照组降低。在CCN中,研究组左侧DLPFC与左侧枕上回的FC较对照组降低,右侧DLPFC与左侧枕上回、右侧矩状回的FC较对照组降低。相关性分析显示,研究组右侧DLPFC与左侧枕上回的FC值与HAMD-24评分呈负相关(P<0.05)。结论 首发抑郁症患者在DMN、SN和CCN均存在FC异常改变,且均与视觉加工皮层的FC异常改变更为明显,本项研究为首发抑郁症患者的临床治疗提供新的脑区关注靶点。Objective To explore the resting-state functional magnetic resonance imaging(rs-fMRI) multi-network brain function alterations and their correlation with clinical symptoms in patients with first-episode depression. Methods A total of 22 patients with first-episode depression(study group) and 18 healthy controls(control group) were collected, and rs-fMRI scans were performed on all subjects in both groups, respectively, with the posterior cingulate gyrus(PCC) as the seed point of default mode network(DMN), the anterior insula(INS) as the seed point of salience network(SN), and the dorsolateral prefrontal lobe(DLPFC) as the seed point of cognitive control network(CCN). The differences between two groups in the DMN, SN, and CCN were observed using seed point-based functional connectivity(FC) analysis. Subjects in study group were assessed with Hamilton Depression Scale-24 items(HAMD-24) and Hamilton Anxiety Scale(HAMA) to explore the correlations between differential brain regions and clinical symptoms. Results In the DMN, FCs in the left PCC with the right inferior occipital gyrus and in the right PCC with the left angular gyrus in study group were reduced compared with those in control group. In the SN, FCs of the left anterior insula with the left inferior temporal gyrus and the right superior occipital gyrus in study group were reduced compared with those in control group. In the CCN, the FC of the left DLPFC with the left superior occipital gyrus was decreased in study group, and the FCs of the right DLPFC with the left superior occipital gyrus and the right momentary gyrus were decreased in study group compared with those in control group. Correlation analysis showed that the FC value of right DLPFC with left supraoccipital gyrus in study group was negatively correlated with the HAMD-24 score(P<0.05). Conclusion Patients with first-episode depression have abnormal FC alterations in the DMN, SN and CCN, and all are more pronounced with abnormal FC alterations in the visual processing cortex.
