扶正通络汤辅助治疗缺血性脑卒中后偏瘫的效果观察

摘　　要：目的研究扶正通络汤辅助治疗缺血性脑卒中后偏瘫的效果。方法队列研究。抽取2021年1月至2022年12月漯河市中医院收治的缺血性脑卒中后偏瘫患者86例,按随机数字表法分为单一组和联合组,每组43例。单一组采用常规西药治疗,联合组在单一组基础上采用扶正通络汤治疗。比较两组治疗效果,比较两组治疗前后炎症因子[白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)]水平、神经相关因子[神经元特异性醇化酶(NSE)、脑源性神经营养因子(BDNF)]水平、肢体功能[Fugl-Meyer运动功能评分表(FMA)]及肌张力[改良Ashworth肌张力(MAS)评分]。结果联合组总有效率(95.35%,41/43)高于单一组(74.42%,32/43),P<0.05。治疗后,联合组IL-1β、TNF-α水平低于单一组(P<0.05);治疗后,联合组NSE水平低于单一组,BDNF水平高于单一组(P<0.05);治疗后,联合组上肢功能、下肢功能评分高于单一组,MAS评分低于单一组(P<0.05)。结论扶正通络汤联合常规西药治疗缺血性脑卒中后偏瘫可提升临床疗效,减轻机体炎症,调节神经相关因子,改善患者肢体功能及肌张力。Objective To study the effect of Fuzheng Tongluo decoction in adjuvant treatment of hemiplegia after ischemic stroke.Methods A total of 86 patients with hemiplegia after ischemic stroke treated in Luohe Hospital of Traditional Chinese Medicine from January 2021 to December 2022 were selected for the cohort study.And they were divided into a single group and a combined group by random number table method,with 43 cases in each group.The single group was treated with conventional western medicine,and the combined group was treated with Fuzheng Tongluo decoction based on the treatment of the single group.The curative effect of the two groups were compared.The levels of inflammatory factors,including interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α),nerve related factors,including neuron specific enolase(NSE)and brain-derived neurotrophic factor(BDNF),limb function assessed by Fugl-Meyer assessment(FMA),and muscle tone assessed by movement assessment scale(MAS)were compared between the two groups before and after treatment.Results The total effective rate of the combined group was 95.35%(41/43),which was higher than the 74.42%(32/43)of the single group(P<0.05).After treatment,the levels of IL-1βand TNF-αin the combined group were lower than those in the single group(P<0.05).After treatment,compared with the single group,the combined group had a lower NSE level and a higher BDNF level(P<0.05).After treatment,compared with the single group,the scores of upper limb function and lower limb function in the combined group were higher,and the MAS score in the combined group was lower(P<0.05).Conclusions Fuzheng Tongluo decoction combined with conventional western medicine in the treatment of hemiplegia after ischemic stroke can improve the clinical efficacy,reduce inflammation,regulate nerve related factors,improve limb function and muscle tone.
