自拟中药外洗方加黑豆配合派瑞松乳膏治疗鹅掌风的临床效果

摘　　要：目的探讨自拟中药外洗方加黑豆配合派瑞松乳膏治疗鹅掌风的临床效果。方法选取2020年6月至2023年4月济南市章丘区中医医院皮肤科收治的共计126例鹅掌风患者作为研究对象,按照随机数字表法分为中药组(42例)、常规组(42例)、联合组(42例)。中药组给予自拟中药外洗方加黑豆治疗,常规组给予派瑞松乳膏治疗,联合组给予自拟中药外洗方加黑豆配合派瑞松乳膏治疗。比较三组的临床疗效、症状体征评分、生活质量[皮肤病生活质量指数问卷(DLQI)]、真菌清除率。结果联合组患者的治疗总有效率、真菌清除率高于中药组及常规组,差异有统计学意义(P<0.017),中药组患者的治疗总有效率、真菌清除率与常规组比较,差异无统计学意义(P>0.017);三组患者治疗后的症状体征(皮损面积、红斑、瘙痒、脱屑)评分、DLQI评分低于本组治疗前,联合组患者治疗后的评分低于中药组、常规组,差异有统计学意义(P<0.05),中药组患者治疗后的评分与常规组比较,差异无统计学意义(P>0.05)。结论自拟中药外洗方加黑豆配合派瑞松乳膏应用于鹅掌风患者治疗中,能够提高临床疗效,降低症状体征评分,改善生活质量,提高真菌清除率。Objective To explore the clinical effect of using a self-designed Chinese medicine external washing formula and black beans and Pevisone Cream in patients with tinea manuum.Methods A total of 126 patients with tinea manuum admitted to Department of Dermatology,Jinan Zhangqiu District Hospital of Traditional Chinese Medicine from June 2020 to April 2023 were selected as the study objects.They were divided into the traditional Chinese medicine(TCM)group(42 cases),the conventional group(42 cases),and the combination group(42 cases)according to random number table method.The TCM group was treated with a self-designed Chinese medicine external washing formula and black beans,the conventional group was treated with Pevisone Cream,and the combination group was treated with self-designed Chinese medicine external washing formula and black beans and peamethasone cream.The clinical efficacy,symptom and sign scores,dermatology quality of life(DLQI),and fungal clearance rate among three groups of patients were compared.Results The total effective rate of treatment,fungal clearance rate of the combination group was higher than those of the TCM group and the conventional group,and the differences were statistically significant(P<0.017).In the total effective rate of treatment and fungal clearance rate,there were no statistically significant differences between the TCM group and the conventional group(P>0.017).The symptom and sign scores(skin lesion area,erythema,itching,and scaling),DLQI scores of the three groups of patients after treatment were lower than those before treatment,and the scores of the combined group patients after treatment were lower than those of the TCM group and the conventional group,and the differences were statiscally significant(P<0.05).There were no significant differences between the scores of the TCM group patients after treatment and the conventional group(P>0.05).Conclusion The application of self-designed TCM external washing formula and black beans and Pevisone Cream in the treatment of patients
