摘  要:目的了解云南省某三级甲等儿童医院临床医护人员职业倦怠、医患关系及离职意愿的相关性,分析医患关系在离职意愿与职业倦怠间的中介作用,为管理者制定策略以预防和缓解临床医护人员职业倦怠提供一定的指导和依据。方法采用便利抽样法,2023年1—3月对云南省某三级甲等儿童医院1273名临床医护人员利用职业倦怠量表、医患关系量表和离职意愿量表进行调查,并对数据进行分析。结果儿童医院临床医护人员的职业倦怠得分为(41.27±22.08)分,医患关系得分为(21.93±6.68)分,离职意愿得分为(12.21±4.00)分。不同学历、岗位、职务、工作科室、月均收入临床医护人员职业倦怠得分比较,差异均有统计学意义(均P<0.05);不同岗位、工作科室临床医护人员医患关系得分比较,差异均有统计学意义(均P<0.05);不同性别、学历、岗位、职称、工作科室、聘用形式临床医护人员离职意愿得分比较,差异均有统计学意义(均P<0.05)。医患关系、离职意愿与职业倦怠总分及情绪衰竭维度、去人格化维度均呈正相关(均P<0.01),与个人成就感维度均呈负相关(均P<0.01);医患关系与离职意愿呈正相关(P<0.01)。医患关系在儿童医院临床医护人员离职意愿与职业倦怠中起部分中介作用,中介效应为0.725,占总效应的38%。结论云南省某三级甲等儿童医院临床医护人员的离职意愿越强烈,医患关系越困难,职业倦怠越严重,医患关系在离职意愿和职业倦怠中起部分中介作用,管理部门应当营造良好和谐的医患大环境,缓解医患关系紧张现状,尽可能为临床医护人员提供舒适良好的执业环境,重点关注医护人员的心理状况和离职意愿并及时给予心理疏导和干预措施,以降低职业倦怠的发生发展。Objective To understand the correlation between job burnout,doctor-patient relationship and turnover intention among clinical medical staff in a grade-A tertiary children's hospital in Yunnan Province,and analyze the mediating role of doctor-patient relationship between turnover intention and job burnout,so as to provide some guidance and basis for managers to formulate strategies to prevent and alleviate job burnout among clinical medical staff.Methods Using the convenient sampling method,1273 clinical medical staff in a grade-A tertiary children's hospital in Yunnan Province were investigated by using job burnout scale,doctor-patient relationship scale and turnover intention scale from January to March 2023,and the data was analyzed.Results The score of job burnout of clinical medical staff in children's hospital was(41.27±22.08)points,the score of doctor-patient relationship was(21.93±6.68)points,and the score of turnover intention was(12.21±4.00)points.There were significant differences in job burnout scores among clinical medical staff with dfferent educational backgrounds,post,positions,working departments and monthly income(all P<0.05).There were significant differences in doctor-patient relationship scores among clinical medical staff with different positions and departments(both P<0.05).There were significant differences in the scores of turnover intention of clinical medical staff with different gender,education background,post,professional title,working department and employment form(all P<0.05).Correlation analysis showed that the doctor-patient relationship and turnover intention were positively correlated with the total score of job burnout,the dimensions of emotional exhaustion and depersonalization(all P<0.01),and negatively correlated with the dimensions of personal accomplishment(both P<0.01).The doctor-patient relationship was positively correlated with turnover intention(P<0.01).The analysis of the mediating effect showed that the doctor-patient relationship plays a partial mediating role i
