润眼明目方熏蒸联合玻璃酸钠滴眼液治疗睑板腺功能障碍致干眼症临床研究

摘　　要：目的:观察润眼明目方熏蒸联合玻璃酸钠滴眼液(HYCOSAN)治疗睑板腺功能障碍(MGD)致干眼症患者的临床疗效。方法:选取MGD致干眼症患者112例,随机分为对照组和治疗组各56例。对照组采用HYCOSAN治疗,治疗组在对照组基础上联合润眼明目方熏蒸治疗。治疗4周后,比较2组临床疗效,以及2组治疗前后泪膜破裂时间(BUT)、角膜荧光素染色评分(FLS)、中医证候评分。结果:治疗后,治疗组总有效率为89.29%,对照组为75.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组BUT均较治疗前增加(P<0.05),FLS均较治疗前降低(P<0.05);且治疗组BUT长于对照组(P<0.05),FLS低于对照组(P<0.05)。治疗后,2组眼干涩、畏光、腰膝酸软、头晕耳鸣、夜寐多梦等中医证候评分均较治疗前降低(P<0.05),且治疗组各项评分均低于对照组(P<0.05)。结论:润眼明目方熏蒸联合HYCOSAN治疗睑板腺功能障碍致干眼症,能够有效缓解患者中医证候,提高临床总有效率,改善患者睑板腺功能。Objective:To observe the clinical effect of fumigation with Runyan Mingmu Prescription combined with Sodium Hyaluronate Eye Drop(HYCOSAN)for patients with xerophthalmia caused by meibomian gland dysfunction(MGD).Methods:A total of 112 cases of patients with MGD-induced xerophthalmia were randomly divided into the control group and the treatment group,with 56 cases in each group.The control group was treated with HYCOSAN,and the treatment group was additionally treated with fumigation with Runyan Mingmu Prescription based on the treatment of the control group.After four weeks of treatment,the clinical effects,Tear Film Break Up Time(BUT),Corneal fluorescein staining scores(FLS)and traditional Chinese medicine(TCM)syndrome scores were compared between the two groups.Results:After treatment,the total effective rate was 89.29%in the treatment group and 75.00%in the control group,the difference being significant(P<0.05).After treatment,BUT in the two groups was increased when compared with that before treatment(P<0.05),and FLS was decreased when compared with that before treatment(P<0.05);BUT in the treatment group was longer than that in the control group(P<0.05),and FLS was lower than that in the control group(P<0.05).After treatment,the scores of TCM syndromes such as dry eyes,photophobia,soreness and weakness of waist and knees,dizziness and tinnitus,and profuse dreaming in the two groups were decreased when compared with those before treatment(P<0.05),and the above scores in the treatment group were lower than those in the control group(P<0.05).Conclusion:The application of fumigation with Runyan Mingmu Prescription combined with HYCOSAN for patients with MGD-induced xerophthalmia can effectively relieve their TCM symptoms,enhance the total clinical effective rate and improve the meibomian gland functions.
