摘　　要：目的研究腹泻型肠易激综合征(diarrhea-predominant irritable bowel syndrome,IBS-D)患者胃动素(motilin,MTL)、脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)表达水平与病情严重程度的关系。方法选取102例我院于2020年8月至2022年8月收治的IBS-D患者作为研究组,另选取102名于同期在我院进行体检的健康受试者作为健康对照组。评估两组患者症状严重程度及内脏敏感度;采用酶联免疫吸附法测定血清中MTL、BDNF水平。分析患者血清MTL、BDNF水平及其与疾病严重程度的关系。结果与健康对照组相比,研究组血清MTL水平较高、BDNF水平较低,且具有较高的内脏敏感度(P<0.05)。患者血清MTL和BDNF水平与疾病严重程度分别呈正相关和负相关(P<0.05)。多因素分析结果显示,内脏敏感指数(visceval sensitivity index,VSI)、MTL是IBS-D患者病情严重程度的独立危险因素,客观内脏敏感度评分、BDNF是IBS-D患者病情严重程度的独立保护因素。结论IBS-D患者血清MTL呈高表达,BDNF呈低表达,与疾病严重程度具有明显相关性,可用于临床判断患者疾病的严重程度。Objective To investigate the correlation between motilin(MTL)and brain-derived neurotrophic factor(BDNF)expression levels and the severity of diarrhea-predominant irritable bowel syndrome IBS-D.Methods A total of 102 patients with IBS-D,who were treated at our hospital from Aug.2020 to Aug.2022,were selected as the research group.Additionally,102 healthy individuals who underwent physical examinations at our hospital during the same period were chosen as the healthy control group.Symptom severity and visceral sensitivity of both groups were evaluated,and the levels of MTL and BDNF in serum were measured using enzyme-linked immunosorbent assay.The relationship between serum MTL and BDNF levels and the severity of the disease were analyzed.Results In comparison to the healthy control group,the study group exhibited higher serum MTL levels,lower BDNF levels,and increased visceral sensitivity(P<0.05).Furthermore,the levels of serum MTL and BDNF were found to be positively and negatively correlated with the severity of the disease,respectively(P<0.05).Multivariate analysis indicated that visceral sensitivity index(VSI)and MTL were independent risk factors,while the objective visceral sensitivity score and BDNF were identified as independent protective factors for the severity of IBS-D.Conclusion Patients with IBS-D exhibit high serum MTL expression and low BDNF expression,which are significantly associated with the severity of the disease.These findings suggest that these expressions can be clinically utilized to assess the severity of the disease.
