摘　　要：目的分析并探讨CK20及Cathepsin K在肾脏具有嗜酸细胞形态肿瘤中的表达及意义。方法回顾性分析155例肾脏肿瘤,具有嗜酸细胞形态的包括:34例嫌色性肾细胞癌嗜酸细胞亚型、17例肾脏嗜酸细胞瘤、16例小眼畸形相关转录因子家族易位性肾细胞癌、6例嗜酸性实性囊性肾细胞癌、5例肾脏上皮样血管平滑肌脂肪瘤、4例肾脏低级别嗜酸细胞肿瘤、3例延胡索酸水合酶缺陷型肾细胞癌、6例未能分类的嗜酸性肾肿瘤。并将44例I型乳头状肾细胞癌、20例透明细胞性肾细胞癌作为对照,观察CK20及Cathepsin K在上述肿瘤中的表达情况。结果CK20及Cathepsin K在上述肿瘤中表达结果基本相似,在多种嗜酸细胞形态肾肿瘤中呈现高阳性率,在对照组中呈现阴性及低表达。结论CK20及Cathepsin K在诊断肾脏伴有嗜酸细胞形态的肿瘤时,缺乏特异性,并不能作为包括嗜酸性实性囊性肾细胞癌在内的具体某一类型的特异性标记,但两个抗体同时阳性能够对嗜酸细胞形态的肾肿瘤初步分类,有助于进一步分析和诊断。Objective To investigate the expression and significance of CK20 and Cathepsin K in renal eosinophilic tumors.Methods Retrospective analysis of 155 cases of renal tumors with eosinophilic cell morphology was conducted.There were 34 chromophobic renal cell carcinoma eosinophilic subtypes,17 renal eosinophilic tumors,16 translocative renal cell carcinoma associated with microocular malformation,6 eosinophilic solid cystic renal cell carcinoma,5 renal epithelioid angiomyolipoma,4 renal low-grade eosinophilic tumors,3 renal cell carcinoma with fumarate hydratase deficiency,and 6 unclassified eosinophilic tumors.The expression of CK20 and Cathepsin K in 44 papillary renal cell carcinoma type I and 20 clear cell renal cell carcinoma was studied as controls.Results The expression of CK20 and Cathepsin K was similar in these tumors,showing high positive rates in multiple eosinophilic renal tumors,and negative and low expression in control group.Conclusion CK20 and Cathepsin K lack specificity in the diagnosis of renal tumors associated with eosinophilic morphology and cannot be used as specific markers for a specific type including eosinophilic solid cystic renal cell carcinoma.However,the simultaneous positive of both antibodies can provide a preliminary classification of eosinophilic renal tumors and facilitate further analysis and diagnosis.
