摘 要:目的研究脑小血管病中APOE4携带与认知及步态的相关性。方法本研究共纳入119例脑小血管病患者,对这些患者进行APOE4基因分型、认知评估、单任务和双任务步态分析,并对上述结果进行统计学分析明确APOE4、认知和步态的关系。结果脑小血管病患者中,APOE4携带与否与步态及认知均无关联(P>0.05);不论是单任务步行还是双任务步行中,认知功能越差,步幅、步速、足趾离地角度平均值、足跟着地角度平均值越小;认知功能的状态与步幅、步速、足趾离地角度平均值、足跟着地角度平均值均有关(P<0.001),经逐步法的单因素和多因素线性回归分析后,上述步态参数仍然与认知有关(P<0.001)。结论脑小血管病患者的认知功能可影响步态,认知是脑小血管病步态的标志物之一。Objective To study the correlation of APOE 4 carriage with cognition and gait in cerebrovascular disease.Methods A total of 119 patients with cerebral small vessel disease were included in this study.APOE4 genotyping,cognitive assessment,single-task and dual-task gait analysis were performed on these patients,and the above results were statistically analyzed to clarify the relationship between APOE4,cognition,and gait.Results Our study found that the presence or absence of APOE4 was not associated with gait and cognition in patients with cerebral small vessel disease(P>0.05).Whether it is single-task walking or dual-task walking,the poorer the cognitive function,the smaller the stride,pace,average toe off ground angle,and average heel landing angle.The state of cognitive function is related to stride length,stride speed,average toe off ground angle,and average heel landing angle(P<0.001).The state of cognitive function is related to stride length,stride speed,average toe off ground angle,and average heel landing angle(P<0.001).After univariate and stepwise multivariate linear regression analysis,the above gait parameters are still related to cognition(P<0.001).Conclusion Cognitive function in patients with cerebral small vessel disease can affect gait.And cognition is one of the markers of gait in cerebral small vessel disease.
