摘  要:目的:探讨颅脑磁共振在非痴呆型血管性认知障碍(VCIND)早期诊断中的价值。方法:选取2017年6月—2019年6月衡水市第五人民医院收治的脑卒中患者116例为观察组,根据认知功能分为障碍组(无痴呆认知功能障碍,n=58)和正常组(认知功能正常,n=58),另选取58例健康志愿者为对照组。三组均接受认知功能评定量表(LOTCA)、精神状态检查量表(MMSE)评估及颅脑磁共振检查。比较三组的认知功能、精神状态和N-乙酰天门冬氨酸甲酯(NAA)/肌酸(Cr)、胆碱复合物(Cho)/Cr水平,分析颅脑磁共振与NAA/Cr、Cho/Cr的相关性。结果:障碍组LOTCA评分低于对照组与正常组,MMSE评分低于对照组,差异有统计学意义(P<0.05)。障碍组左侧、右侧NAA/Cr水平低于对照组与正常组,左侧Cho/Cr水平高于对照组与正常组,右侧Cho/Cr水平高于对照组,差异有统计学意义(P<0.05);正常组左侧、右侧NAA/Cr水平低于对照组,左侧、右侧Cho/Cr水平高于对照组,差异有统计学意义(P<0.05)。Pearson相关分析显示,障碍组LOTCA评分与左侧、右侧Cho/Cr水平呈负相关(P<0.05),与左侧、右侧NAA/Cr水平呈正相关(P<0.05)。结论:颅脑磁共振在VCIND早期诊断中的价值较高,可为血管性认知障碍的早期诊断及分型提供依据。Objective:To investigate the value of craniocerebral magnetic resonance in the early diagnosis of non-dementia vascular cognitive impairment(VCIND).Methods:A total of one hundred and sixteen stroke patients admitted to Hengshui Fifth People's Hospital from June 2017 to June 2019 were selected as the observation group.According to cognitive function,they were divided into the disabled group(no dementia cognitive dysfunction,n=58)and the normal group(normal cognitive function,n=58).Another fifty-eight healthy volunteers were selected as control group.All three groups underwent cognitive function rating scale(LOTCA),mental state examination scale(MMSE)and brain magnetic resonance examination.The cognitive function,mental status and levels of N-acetyl-aspartate methyl ester(NAA)/creatine(Cr)and choline complex(Cho)/Cr were compared among the three groups.The correlation between brain magnetic resonance and NAA/Cr,Cho/Cr was analyzed.Results:The LOTCA score of the disabled group was lower than that of the control group and the normal group,and the MMSE score was lower than that of the control group,the difference was statistically significant(P<0.05).The NAA/Cr level on the left and right side of the disabled group was lower than that of the control group and normal group,the Cho/Cr level on the left was higher than that of the control group and normal group,and the Cho/Cr level on the right was higher than that of the control group,the difference was statistically significant(P<0.05);the left and right NAA/Cr levels in normal group were lower than those in control group,and the left and right Cho/Cr levels were higher than those in control group,the difference was statistically significant(P<0.05).Pearson correlation analysis showed that LOTCA score was negatively correlated with Cho/Cr level on left and right side(P<0.05),and positively correlated with NAA/Cr level on left and right side(P<0.05).Conclusion:Craniocerebral magnetic resonance is of high value in the early diagnosis of VCIND,which can provide basis for
