异种脱细胞真皮基质组织补片置入对腮腺良性肿瘤患者术后味觉出汗综合征发生的影响

摘　　要：目的探讨异种脱细胞真皮基质组织补片(H-AMD)置入对腮腺良性肿瘤患者术后味觉出汗综合征(Frey综合征)的影响,以期为临床预防腮腺良性肿瘤患者术后发生Frey综合征提供参考。方法选取2018年3月—2021年12月在成都市中西医结合医院接受手术切除治疗的90例腮腺良性肿瘤患者,按照是否置入H-AMD分为对照组(44例)与研究组(46例)。比较2组患者手术及术后恢复相关指标、术区异物感及其他并发症发生情况、Frey综合征发生情况、美容满意度等。结果2组患者术中出血量、手术时间、引流管放置时间、住院时间比较,差异均无统计学意义(P>0.05)。2组患者术后术区异物感及面瘫、耳周麻木、唾液漏、感染、发热等并发症发生率比较,差异均无统计学意义(P>0.05)。研究组患者术后12个月内Frey综合征主观判定阳性率与客观判定阳性率均低于对照组,差异均有统计学意义(P<0.05)。研究组患者术后3个月、6个月时Frey综合征客观判定阳性率低于对照组,差异均有统计学意义(P<0.05);2组患者术后12个月时Frey综合征客观判定阳性率比较,差异无统计学意义(P>0.05)。研究组患者美容满意度优于对照组,差异有统计学意义(P<0.05)。结论H-AMD置入可有效降低腮腺良性肿瘤患者术后Frey综合征发生风险,提高患者美容满意度,且不会造成患者不适或增加其他并发症发生风险。Objective To investigate the effect of heterogeneous acellular dermal matrix(H-ADM)implantation on Frey's syndrome(gustatory sweating)in patients with benign parotid tumors after surgery and to provide a clinical reference for prevention.Methods From March 2018 to December 2021,90 patients who underwent parotid tumor excision were assigned to control group(44 patients)or study group(46 patients)based on H-ADM implantation.Surgery and postoperative recovery,foreign body sensation,complications,Frey's syndrome occurrence,and cosmetic satisfaction were compared between the groups.Results No significant differences were found in intraoperative blood loss,time of operation,time of drainage tube placement,or length of hospital stay between the groups(P>0.05).Incidence of postoperative foreign body sensation,facial paralysis,ear numbness,salivary leakage,infection,and fever were also not significantly different(P>0.05).The study group showed significantly lower subjective and objective positive rates of Frey's syndrome within 12 months after surgery compared to the control group(P<0.05).At 3 and 6 months after surgery,the objective positive rate in the study group was significantly lower(P<0.05),but there was no significant difference at 12 months(P>0.05).Cosmetic satisfaction was significantly higher in the study group than in the control group(P<0.05).Conclusion H-ADM implantation effectively reduces the risk of Frey's syndrome in patients with benign parotid tumors after surgery,elevates cosmetic satisfaction,and does not increase discomfort or the risk of other complications.
