探讨导丝制作的圈套器治疗胃柿石的临床疗效

摘　　要：目的分析在胃柿石治疗期间,导丝制作的圈套器治疗方案应用效果。方法在2013年7月至2022年7月期间本院收治的胃柿石患者中选取80例行对比调查,依据治疗方案的不同分组,其中40例行传统异物钳与圈套器等捣碎治疗纳入到对照组,另40例行导丝制作的圈套器碎石治疗纳入观察组,对比不同方案应用效果。结果观察组患者平均治疗操作时间、平均住院时间低于对照组(P<0.05)。观察组患者1次治疗成功率高于对照组(P<0.05)。两组患者上腹部沉坠感与反酸症状改善时间上无差异性(P>0.05),且观察组患者恶心呕吐、腹胀症状改善时间短于对照组(P<0.05)。治疗前与治疗后3 d,两组患者疼痛评分无差异性(P>0.05),且治疗后1 h与1 d,观察组患者疼痛评分低于对照组(P<0.05)。治疗前,两组患者生活质量评分无差异性(P>0.05);治疗后,所有患者均有改善,除物质生活之外,其他各生活质量指标评分均以观察组较高(P<0.05)。结论胃柿石的治疗以碎石为主,可采取多种方法完成碎石操作,与传统常用工具对比,导丝制作的圈套器完成碎石操作的时间更短,有助于缩短患者住院时间,且碎石效果更为理想,可降低患者所承受的痛苦。Objective To analyze the application effect of a snare treatment plan made of guide wire during the treatment of stomach persimmon stone.Methods From July 2013 to July 2022,80 patients with stomach persimmon stone to our hospital were selected for comparative investigation.According to different treatment regiments,40 of them were included in the control group through traditional foreign body forceps and snare crushing,and the other 40 were included in the observation group through snare lithotripsy made by wire guide,and the application effects of different regiments were compared.Results The average operation time and the average hospital stay in the observation group were lower than those in the control group(P<0.05).The success rate of one treatment in observation group was higher than that in control group(P<0.05).There was no difference in the improvement time of upper abdominal sinking and acid reflux symptoms between the two groups(P>0.05),and the improvement time of nausea,vomiting and abdominal distension in observation group was shorter than that in control group(P<0.05).Before treatment and 3 days after treatment,there was no difference in pain score between the two groups(P>0.05),and 1 h and 1 d after treatment,the pain score of the observation group was lower than that of the control group(P<0.05).Before treatment,there was no difference in quality of life scores between the two groups(P>0.05).After treatment,all patients were improved,except for material life,the scores of other quality of life indexes were higher in observation group(P<0.05).Conclusions The treatment of stomach persimmon stone is mainly lithotripsy,and various means can be taken to complete the lithotripsy operation.Compared with traditional common tools,the trap made of guide wire has a shorter time,which helps to shorten the hospital stay of patients,and the effect of lithotripsy is more ideal,which can reduce the pain of patients.
