摘　　要：目的:分析微生物检验对肺部曲霉菌感染的诊断价值。方法:选取2020年11月—2024年1月天水市中西医结合医院收治的疑似肺部曲霉菌感染患者100例为研究对象,患者均实施痰培养、(1-3)-β-D-葡聚糖(G)检测试验(简称G试验)、曲霉半乳甘露聚糖(GM)抗原检测试验(简称GM试验)。以病理检查结果为“金标准”,分析微生物检验对肺部曲霉菌感染的诊断价值。结果:病理确诊阳性88例,阴性12例;痰培养检出阳性74例,阴性26例;G试验检出阳性78例,阴性22例;GM试验检出阳性81例,阴性19例;联合检验检出阳性87例,阴性13例。联合检验的灵敏度、准确率高于痰培养,差异有统计学意义(P<0.05);联合检验与G试验、GM试验的灵敏度、特异度、准确率比较,差异无统计学意义(P>0.05);联合检验与痰培养的特异度比较,差异无统计学意义(P>0.05)。结论:痰培养、G试验、GM试验联合检验在肺部曲霉菌感染诊断中的价值较高,可提高诊断灵敏度及准确率。Objective:To analyze the diagnostic value of microbiological test for pulmonary aspergillus infection.Methods:A total of one hundred patients with suspected pulmonary aspergillus infection admitted to Tianshui Hospital of Integrated Traditional Chinese and Western Medicine from November 2020 to January 2024 were selected as the study objects.Sputum culture,(1-3)-β-D-glucan(G)test and Aspergillus galactomannan(GM)antigen test were performed in all patients.The value of microbiological examination in the diagnosis of pulmonary aspergillus infection was analyzed with the results of pathological examination as the"gold standard".Results:88 cases were confirmed positive and 12 cases were confirmed negative by pathology;sputum culture was positive in 74 cases and negative in 26 cases;G test detected 78 positive cases,22 negative cases;GM test was positive in 81 cases and negative in 19 cases;the combined test detected 87 positive cases and 13 negative cases.The sensitivity and accuracy of combined test were higher than that of sputum culture,and the difference was statistically significant(P<0.05);there was no significant difference in sensitivity,specificity and accuracy between the combined test,G test and GM test(P>0.05);there was no significant difference in the specificity between combined test and sputum culture(P>0.05).Conclusion:The combined test of sputum culture,G test and GM test is of high value in the diagnosis of pulmonary aspergillus infection,which can improve the diagnostic sensitivity and accuracy.
