高速涡轮钻微创法、传统劈冠法治疗下颌低位水平埋伏阻生牙的临床应用对比研究

摘  要:目的:对比分析高速涡轮钻微创法、传统劈冠法治疗下颌低位水平埋伏阻生牙的临床应用效果。方法:回顾性收集2020年7月至2022年7月期间,且进行1 y随访于本院收治的下颌低位水平埋伏阻生牙113例患者的临床资料。根据治疗方案的差异将患者分为对照组(采用传统劈冠法治疗,58例)和观察组(采用高速涡轮钻微创法,55例)。分析术中、术后两组的手术相关指标(手术时间、术后伤口感染率、采用视觉模拟量表(Visual analogue scale,VAS)评分评估疼痛情况)、治疗后,评价两组恐惧适应度及手术安全性,通过随访复查,经数字量化法和手指量化法观察并比较两组术后3 m、9 m、12 m完全性张口受限情况。结果:观察组的手术时间短于对照组,VAS评分显著低于对照组(P<0.05);两组术后伤口感染率比较差异无统计学意义(P>0.05)。观察组0°恐惧例数显著高于对照组,I°及Ⅱ°恐惧例数显著低于对照组(P<0.05)。观察组手术并发症总发生率显著低于对照组(P<0.05)。观察组术后3 m、6 m完全性张口受限率低于对照组(P<0.05),两组12 m完全性张口受限率比较差异无统计学意义(P>0.05)。结论:高速涡轮钻微创法能改善患者恐惧心理,缓解疼痛,缩短手术时间,减少术后并发症发生,改善术后张口受限情况。Objective:To compare and analyze the clinical effect of minimally invasive high-speed turbo-drilling method and traditional split-crown method in the treatment of mandibular low level impacted teeth.Methods:The clinical data of 113 patients with horizontal impacted mandibular teeth treated in our hospital from July 2020 to July 2022 were retrospectively collected.According to the difference of treatment regimen,the patients were divided into control group(58 cases were treated with traditional cleft crown method)and observation group(55 cases were treated with minimally invasive high-speed turbo-drilling method).The operation-related indexes(operation time,postoperative wound infection rate,pain assessment by Visual analogue scale(VAS))of the two groups were analyzed during and after the operation,and the fear adaptability and operation safety of the two groups were evaluated after the treatment.Digital quantification and finger quantification were used to observe and compare the complete mouth opening limitation of the two groups at 3 months,9 months and 12 months after operation.Results:The operation time of observation group was shorter than that of control group,and VAS score was significantly lower than that of control group(P<0.05).There was no significant difference in postoperative wound infection rate between the two groups(P>0.05).The number of 0°fear cases in observation group was significantly higher than that in control group,and the number of I°andⅡ°fear cases was significantly lower than that in control group(P<0.05).The total incidence of surgical complications in observation group was significantly lower than that in control group(P<0.05).The rate of complete mouth opening restriction in the observation group was lower than that in the control group at 3 months and 6 months after operation(P<0.05),and there was no statistical difference between the two groups at 12 months(P>0.05).Conclusion:High-speed turbodrill can improve patients'fear,relieve pain,shorten operation time,reduce postoperati
