间接盖髓术、乳牙活髓切断术治疗乳磨牙深龋近髓的临床近、远期预后对比分析

摘　　要：目的:探究间接盖髓术、乳牙活髓切断术治疗乳磨牙深龋近髓的临床近、远期预后。方法:回顾性收集2021年10月-2023年11月期间本院收治的102例乳磨牙深龋近髓患儿的临床资料。根据治疗方案不同将患儿分为对照组(采用间接盖髓术,50例)和观察组(乳牙活髓切断术,52例)。分析对比两组的临床疗效、近期预后、远期预后以及并发症发生情况。结果:观察组的治疗有效率显著高于对照组(P<0.05)。术后一周,两组的吞咽、语言、咀嚼评分比较均无显著差异(P>0.05)。两组术后3 m的患牙成活率无显著差异(P>0.05);术后6 m,观察组的患牙成活率显著高于对照组(P<0.05)。两组的并发症发生率无显著差异(P>0.05)。结论:相较于间接盖髓术,乳磨牙活髓切断术对乳磨牙深龋近髓患者的临床效果更佳,能提高患者的远期预后,且具有较高的安全性。Objective:To investigate the near and long term prognosis of indirect pulp capping and deciduous teeth living pulp amputation in the treatment of deep caries near pulp of deciduous teeth.Methods:The clinical data of 102 children with deep deciduous teeth and proximal pulp caries admitted to our hospital from October 2021 to November 2023 were retrospectively collected.The children were divided into control group(indirect pulp capping,50 cases)and observation group(live pulp amputation,52 cases)according to different treatment plans.The clinical efficacy,short-term prognosis,long-term prognosis and complications of the two groups were analyzed and compared.Results:The effective rate of the observation group was significantly higher than that of the control group(P<0.05).One week after surgery,there were no significant differences in swallowing,speech and mastication scores between the two groups(P>0.05).There was no significant difference in the survival rate of affected teeth 3 months after surgery between the two groups(P>0.05).After 6 months,the survival rate of affected teeth in the observation group was significantly higher than that in the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Compared with indirect pulp cap,mastomolar live pulp amputation has better clinical effect on patients with deep caries near pulp,can improve the long-term prognosis of patients,and has higher safety.
