摘  要:目的探究重度低龄儿童龋唾液中分泌型免疫球蛋白A(secretory immunoglobulin A,sIgA)、丙二醛(Methylenedioxyamphetamine,MDA)、基质金属蛋白酶-9(matrix metalloprotein-9,MMP-9)的表达水平与意义。方法选取2021年4月—2022年4月于秦皇岛市妇幼保健院口腔科就诊的210例重度低龄儿童龋患儿为研究对象,另选轻度龋病组患儿100例、健康组儿童110例作为对照。通过免疫透射比浊法、ELISA法对三组的sIgA、MDA和MMP-9表达进行检测,并分析sIgA、MDA、MMP-9表达的变化对重度低龄儿童龋的意义。结果与健康儿童相比,轻度、重度龋患儿sIgA、MMP-9表达升高,MDA水平表达降低;与轻度龋相比,重度龋患儿sIgA、MMP-9表达升高,MDA水平表达降低(P<0.05)。唾液中MDA与龋失补牙面(decayed-missing-filled surface,dmfs)指数呈负相关,而sIgA、MMP-9与dmfs指数呈正相关(P<0.05)。sIgA、MDA、MMP-9之间Pearson相关性分析显示,sIgA、MDA之间呈负相关(r=-0.432,P=0.001);sIgA、MMP-9之间呈正相关(r=0.436,P=0.001);MDA、MMP-9之间呈负相关(r=-766,P=0.001)。结论重度低龄儿童龋患儿唾液中sIgA、MMP-9水平升高,MDA水平降低,可看出三者表达对重度低龄儿童龋患儿发病具有重要的意义。Objective To explore the expression level and significance of secreted immunoglobulin A(sIgA),Methylenedioxyamphetamine(MDA)and matrix metalloprotein-9(MMP-9)in the saliva of severe young children.Methods The 210 children with severe low childhood caries admitted to our hospital from April 2021 to April 2022,and 100 children in mild caries group and 110 children in healthy group were selected.The expressions of sIgA,MDA and MMP-9 in the three groups were examined by immunotransmission turbidimetry,ELISA,and the significance of changes in sIgA,MDA and MMP-9 expression for caries in severe younger children was analyzed.Results Compared with healthy children,sIgA and MMP-9 expression were increased compared with healthy children,and severe children had increased sIgA,MMP-9 expression and decreased MDA levels(P<0.05).Salivary MDA was negatively associated with the decayed-missing-filled surface(dmfs)index,while sIgA and MMP-9 were positively associated with the dmfs index(P<0.05).The Pearson correlation analysis between sIgA,MDA,and MMP-9 showed a negative correlation between sIgA and MDA(r=-0.432,P=0.001),a positive correlation between sIgA and MMP-9(r=0.436,P=0.001),and a negative correlation between MDA and MMP-9(r=-766,P=0.001).Conclusion The levels of sIgA,MMP-9 and MDA are increased in children with the presence of caries at severe age.
