全数字化角膜塑形镜与传统角膜塑形镜治疗青少年近视的疗效比较

摘　　要：目的观察全数字化角膜塑形镜(AD-MCT)和传统角膜塑形镜(T-Ortho-k)对青少年近视防控的临床疗效比较。方法纳入2022年7月—2023年1月兰州市普瑞眼科医院就诊佩戴角膜塑形镜的青少年近视患者160例(320只眼),随机分为对照组和观察组,每组80例(160只眼)。对照组配戴T-Ortho-k,观察组配戴AD-MCT。观察指标主要包括双眼裸眼远视力(UCDVA)、眼轴长度及角膜中央光学治疗区直径。结果2组治疗前双眼UCDVA、眼轴长度、角膜中央光学治疗区直径比较,差异均无统计学意义(P>0.05)。(1)UCDVA:治疗后1 d和7 d对照组右眼(OD)和左眼(OS)UCDVA均高于治疗前,差异均有统计学意义(治疗后1 d:t_(OD)=19.840、t_(OS)=23.682,均P=0.000;治疗后7 d:t_(OD)=44.977、t_(OS)=40.046,均P=0.000);治疗后1 d和7 d,观察组UCDVA均高于治疗前,差异均有统计学意义(治疗后1 d:t_(OD)=27.800、t_(OS)=30.789,均P=0.000;治疗后7 d:t_(OD)=38.359、t_(OS)=47.611,均P=0.000)。治疗后2组间比较,治疗后1 d和7 d,观察组UCDVA均高于对照组,差异均有统计学意义(治疗后1 d:t_(OD)=5.184、t_(OS)=5.065,均P=0.000;治疗后7 d:t_(OD)=4.498、t_(OS)=4.519,均P=0.000)。(2)眼轴长度及眼轴长度变化值:治疗后1年,对照组眼轴长度均较治疗前增长,差异均有统计学意义(t_(OD)=9.411、t_(OS)=9.796,均P=0.000);观察组眼轴长度也较治疗前增长,差异均有统计学意义(t_(OD)=8.125、t_(OS)=5.906,均P=0.000)。治疗后2组间比较,差异均无统计学差异(均P>0.05)。而治疗后1年观察组眼轴长度变化值较对照组减小,差异均有统计学意义(t_(OD)=-2.808,P=0.006;t_(OS)=-2.738,P=0.007)。(3)角膜中央光学治疗区直径:治疗后1年,观察组角膜中央光学治疗区直径均小于对照组,差异均有统计学意义(t_(OD)=-7.488、t_(OS)=-10.340,均P=0.000)。结论AD-MCT和T-Ortho-k均可有效控制青少年近视发展,AD-MCT的治疗效果优于T-Ortho-k,可能与其更好地缩小角膜中央光学治疗区�OBJECTIVE To compare the clinical efficacy of all-digital myopia corneal therapy lens(AD-MCT)and traditional orthokeratology lenses(T-Ortho-K)in controlling myopia in adolescents.METHODS A total of 160 adolescent myopia patients(320 eyes)who visited Lanzhou Bright Eye Hospital and wore orthokeratology lenses from July 2022 to January 2023 were included and randomly divided into a control group and an observation group,with 80 cases(160 eyes)in each group.The control group wore T-Ortho-K contact lenses,while the observation group wore AD-MCT.Before the treatment,uncorrected distance visual acuity(UCDVA),axial length,and corneal central optical treatment zone diameter of both eyes were measured in both groups.After wearing the lenses for one day and seven days,the UCDVA of both eyes was measured,and the axial length,axial length change value,and corneal central optical treatment zone diameter were measured after one year of treatment.RESULTS There were no statistically significant differences between the two groups in UCDVA,axial length,or corneal central optical treatment zone diameter before treatment(P>0.05).(1)UCDVA:In the control group,UCDVA of both right eye(OD)and left eye(OS)significantly improved one day and seven days after wearing the lenses compared to before treatment(one day after treatment:t_(OD)=19.840,t_(OS)=23.682,both P=0.000;seven days after treatment:t_(OD)=44.977,t_(OS)=40.046,both P=0.000).In the observation group,the UCDVA of both eyes significantly improved one day and seven days after wearing the lenses compared to before treatment(one day after treatment:t_(OD)=27.800,t_(OS)=30.789,both P=0.000;seven days after treatment:t_(OD)=38.359,t_(OS)=47.611,both P=0.000).Comparison between the two groups after treatment showed that the UCDVA of both eyes in the observation group significantly improved compared to the control group one day and seven days after treatment(one day after treatment:t_(OD)=5.184,t_(OS)=5.065,both P=0.000;seven days after treatment:t_(OD)=4.498,t_(OS)=4.519,both P=0.000).
