全自动血球仪检测血常规中红细胞参数在不同年龄段儿童地中海贫血中的诊断意义

摘　　要：目的:分析全自动血球仪检测血常规中红细胞参数在不同年龄段儿童地中海贫血中的诊断意义。方法:选取2022年5月~2023年5月于上犹县人民医院就诊的100例儿童作为研究对象,检测其地中海贫血基因,并根据是否携带地中海贫血基因对其进行分组,其中对照组为未携带地中海贫血基因(80例),观察组为携带地中海贫血基因(20例),均采用同一台五分类全自动血球分析仪进行血常规检测,对比两组血常规指标,分析红细胞参数诊断地中海贫血的效果。结果:与对照组相比,观察组儿童血小板计数、红细胞计数、血小板分布宽度较高,平均红细胞体积、血红蛋白、平均红细胞血红蛋白浓度、平均红细胞血红蛋白含量较低(P<0.05)。平均红细胞体积的受试者操作特征曲线下面积比平均红细胞血红蛋白含量高,在最佳筛查界值下,平均红细胞体积灵敏度最高,血红蛋白特异度最高。结论:平均红细胞体积、血红蛋白等红细胞参数可辅助诊断地中海贫血,提高诊断效果。Objective:To analyze the diagnostic significance of red blood cell parameters detected by automatic blood cell analyzer in children with thalassemia at different ages.Methods:100 children who received medical treatment at Shangyou County People's Hospital from May 2022 to May 2023 were selected as the research subjects.The thalassemia gene was detected and grouped according to whether they carried the thalassemia gene.The control group(80 cases)did not carry the thalassemia gene,and the observation group(20 cases)carried the thalassemia gene.The blood routine tests were carried out by the same five category automatic blood cell analyzer.The blood routine indexes of the two groups were compared,and the effect of red blood cell parameters in the diagnosis of thalassemia was analyzed.Results:Compared with the control group,the observation group had higher platelet count,red blood cell count,platelet distribution width,lower average red blood cell volume,hemoglobin,average red blood cell hemoglobin concentration and average red blood cell hemoglobin content(P<0.05).The area under the operating characteristic curve of the mean corpuscular volume was higher than the mean corpuscular hemoglobin content.Under the optimal screening threshold,the mean corpuscular volume had the highest sensitivity and hemoglobin specificity.Conclusion:The mean red blood cell volume,hemoglobin and other red blood cell parameters can assist in the diagnosis of thalassemia and improve the diagnostic effect.
