单焦点Toric人工晶状体在低度角膜顺规散光白内障患者中的应用效果

摘　　要：目的探讨单焦点Toric人工晶状体在白内障超声乳化联合人工晶状体植入术合并低度角膜顺规散光(WTR)中的应用效果。方法回顾性分析2021年9月至2022年3月于延边大学附属医院行白内障超声乳化吸除联合人工晶状体植入术的合并低度角膜WTR的65例白内障患者的临床资料,根据患者经济条件及本身对视觉质量的需求程度,以患者主观意愿分为Toric组(35例)与对照组(30例)。Toric组行超声乳化吸除联合植入单焦点Toric人工晶状体,对照组行超声乳化吸除联合植入单焦点人工晶状体,比较两组的裸眼远视力(UDVA)、裸眼近视力(UNVA)、残余散光、斯特列尔比值(SR)、对比敏感度、高阶像差以及美国国家眼科研究所视功能量表(NEI-VFQ-25)评分。结果两组患者术后1周及术后1、3个月的UDVA和UNVA均低于本组术前,且Toric组术后1周及术后1、3个月的UNVA、UNVA均低于对照组差异有统计学意义(P<0.05)。Toric组术后3个月的残余散光低于对照组,差异有统计学意义(P<0.05)。术后3个月,Toric组自然光下测量SR高于对照组,差异有统计学意义(P<0.05)。术后3个月,Toric组在18c/d的对比敏感度高于对照组,差异有统计学意义(P<0.05)。术后3个月,两组在自然光小瞳孔下两组间总高阶像差,差异无统计学意义(P>0.05)。Toric组术后3个月的NEI-VFQ-25评分高于对照组,差异有统计学意义(P<0.05)。结论Toric人工晶状体在白内障超声乳化吸除联合人工晶状体植入术的合并低度角膜WTR的白内障患者的治疗中能够更好地改善视觉质量和生存质量。Objective To investigate the application effect of monofocal Toric intraocular lens in phacoemulsification com-bined with intraocular lens implantation with low degree corneal astigmatism with-the-rule(WTR).Methods From Septem-ber 2021 to March 2022,a total of 65 cataract patients with low degree corneal astigmatism WTR who underwent pha-coemulsification combined with intraocular lens implantation in the Yanbian University Hospital were retrospectively ana-lyzed.According to the patients'economic conditions and their own demand for visual quality,they were divided into Toric group(35 cases)and control group(30 cases)according to patients'subjective wishes.The Toric group underwent pha-coemulsification combined with implantation of monofocal Toric intraocular lens,while the control group underwent pha-coemulsification combined with implantation of monofocal intraocular lens.The uncorrected distance visual acuity(UDVA),uncorrected near visual acuity(UNVA),residual astigmatism,Steller's ratio(SR),contrast sensitivity,high-order aberration,and National Eye Institute visual function questionnaire(NEI-VFQ-25)scores were compared between the two groups.Re-sults The UDVA and UNVA of the two groups at 1 week,1 month and 3 months after operation were lower than those before operation,and the UNVA and UNVA of the Toric group at I week,1 month and 3 months after operation were lower than those of the control group,and the differences were statistically significant(P<0.05).The residual astigmatism in the Toric group was lower than that in the control group at 3 months after surgery,and the difference was statistically significant(P<0.05).After 3 months of surgery,the SR measured un-der natural light in the Toric group was higher than that in the control group,and the difference was statistically significant(P<0.05).After 3 months of surgery,the Toric group had a higher contrast sensitivity at 18 c/d than the control group,and the difference was statistically significant(P<0.05).After 3 months of surgery,there was no statistic
