利培酮结合喹硫平治疗癫痫所致精神障碍的价值及对精神状态与血清炎性因子的影响

摘　　要：目的分析利培酮结合喹硫平治疗癫痫所致精神障碍的价值及对精神状态与血清炎性因子的影响。方法随机选取平潭精神病防治院精神科于2020年9月—2022年9月收治的60例癫痫所致精神障碍患者为研究对象,根据随机数表法将患者分成两组,单项治疗组(喹硫平治疗,n=30)与联合治疗组(利培酮结合喹硫平治疗,n=30),对比两组治疗效果、精神状态[简明精神病评定量表(Brief Psychiatric Rating Scale,BPRS)]、社会功能[个人和社会功能量表(Personal and Social Performance Scale,PSP)]、血清炎性因子水平[白细胞介素-1β(Interleukin-1β,IL-1β)、同型半胱氨酸(Homocysteinemia,Hcy)、白介素-12(Interleukin-12,IL-12)]、不良反应。结果联合治疗组治疗总有效率达93.33%,显著高于单项治疗组的70.00%,差异有统计学意义(χ^(2)=5.455,P<0.05)。治疗后两组BPRS评分均呈下降趋势,且联合治疗组下降幅度更大,差异有统计学意义(P<0.05);治疗后联合治疗组PSP评分显著高于单项治疗组,差异有统计学意义(P<0.05)。与治疗前比较,治疗后两组IL-1β、Hcy、IL-12水平均下降,且联合治疗组各指标显著低于单项治疗组,差异有统计学意义(P均<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论利培酮结合喹硫平治疗对癫痫所致精神障碍患者有明显的疗效,可显著改善其精神状态及机体炎症反应,降低血清炎性因子的表达,具有一定的安全性及有效性。Objective To analyze the value of risperidone combined with quetiapine in the treatment of mental disorders caused by epilepsy and the effect on mental state and serum inflammatory factors.Methods Sixty patients with mental disorders caused by epilepsy admitted to the Psychiatric Department of Pingtan Psychiatric Hospital from September 2020 to September 2022 were randomly selected as the research objects,and the patients were divided into two groups according to random number table method.Single treatment group(quetiapine treatment,n=30) and combined treatment group(risperidone combined with quetiapine treatment,n=30).The therapeutic effect,mental status [Brief Psychiatric Rating Scale(BPRS)],social function [Personal and Social Performance Scale(PSP)],serum inflammatory factor levels [interleukin-1β(IL-1β),homocysteine mia(Hcy),interleukin-12(IL-12)] and adverse reactions were compared between the two groups.Results The total effective rate of combined treatment group was 93.33%,which was significantly higher than that of single treatment group(70.00%),and the difference was statistically sig-药物与临床nificant(χ^(2)=5.455,P<0.05).After treatment,the BPRS scores of both groups showed a downward trend,and the decrease was greater in the combined treatment group,the difference was statistically significant(P<0.05);after treatment,PSP score of combined treatment group was significantly higher than that of single treatment group,and the difference was statistically significant(P<0.05).Compared with before treatment,the levels of IL-1β,Hcy and IL-12 in the two groups were all decreased after treatment,and the indexes in the combined treatment group were significantly lower than those in the single treatment group,the differences were statistically significant(all P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Risperidone combined with quetiapine has obvious curative effect on patients with mental disorders caused by epilepsy,
