原发性肝癌介入治疗患者习得性无助感调查、影响因素及护理对策分析

摘　　要：目的对原发性肝癌(PLC)介入治疗患者习得性无助感(LH)进行调查并分析其影响因素,制定护理对策。方法选取2022年5月至2023年12月在安阳市肿瘤医院接受治疗的103例PLC介入治疗患者进行分析,采用习得无助感量表(LHS)调查患者习得性无助感现状,收集其基本资料,并进行单因素分析与多元线性回归分析以明确其影响因素。结果103例PLC介入治疗患者中习得无助感总分为(74.78±5.03)分,其中绝望感得分为(18.76±2.33)分、无助感得分为(56.02±4.18)分;通过Pearson相关性分析,PLC介入治疗患者LHS总分、各维度得分与亲密关系支持问卷(SIRRS)总分、各维度得分呈负相关(P<0.05);多元线性回归分析结果显示,文化程度、家庭月收入、介入治疗次数、肝癌家族史、SIRRS得分是影响PLC介入治疗患者LH的独立危险因素(R^(2)=0.067,调整R^(2)=0.045,P<0.05)。结论PLC介入治疗患者LH水平较高,文化程度、家庭月收入、介入治疗次数、肝癌家族史、感知配偶支持为LH的影响因素,需要实施针对性护理对策,降低LH。Objective To investigate the learned helplessness(LH)in patients with primary liver cancer(PLC)undergoing interventional therapy and analyze its influencing factors,and to formulate nursing strategies.Methods 103 patients with PLC undergoing interventional therapy treated in Anyang Cancer Hospital from May 2022 to December 2023 were selected for analysis.The Learned Helplessness Scale(LHS)was used to investigate the current situation of learned helplessness in patients.Their basic information was collected,and univariate analysis and multiple linear regression analysis were conducted to clarify its influencing factors.Results Among the 103 patients with PLC undergoing interventional therapy,the total score of learned helplessness was(74.78±5.03)points,of which the score of despair was(18.76±2.33)points,and the score of helplessness was(56.02±4.18)points.Through Pearson correlation analysis,the total score and each dimension score of LHS in patients with PLC undergoing interventional therapy were negatively correlated with the total score and each dimension score of the Support Inventory for Romantic Relationships(SIRRS)(P<0.05).The results of multiple linear regression analysis showed that education level,family monthly income,number of interventional treatments,family history of liver cancer,and SIRRS score were independent risk factors affecting LH in patients with PLC undergoing interventional therapy(R^(2)=0.067,adjusted R^(2)=0.045,P<0.05).Conclusion The level of LH in patients with PLC undergoing interventional therapy is relatively high.Education level,family monthly income,number of interventional treatments,family history of liver cancer,and perceived spousal support are influencing factors of LH.It is necessary to implement targeted nursing strategies to reduce LH.
