摘 要:背景:窄带成像技术可以提高结直肠息肉检出率(PDR)。然而,结肠镜中的光学增强模式较高清白光成像(HDWL)是否可增加PDR尚不清楚。目的:探讨光学增强电子染色技术在结直肠息肉筛查中的应用价值。方法:回顾性分析2020年8月—2023年11月期间于河北省邢台市人民医院接受非镇静结肠镜检查的371例患者。根据退镜时使用的模式,将患者分为HDWL组和光学增强组。比较两组PDR和腺瘤检出率(ADR)。结果:本研究共纳入了371例患者行结肠镜检查,其中HDWL模式199例,光学增强模式172例。HDWL组和光学增强组之间的性别、年龄、肠道准备评分相比无明显差异(P>0.05)。光学增强组的PDR(57.0%对42.2%,P=0.005)、ADR(25.6%对14.6%,P=0.008)明显高于HDWL组。性别亚组分析结果显示,在男性患者中,光学增强组的PDR(65.9%对45.9%,P=0.007)和ADR(31.8%对17.3%,P=0.023)显著高于HDWL组。结论:在结肠镜检查中,常规使用光学增强模式可显著改善PDR和ADR。Background:Narrow⁃band imaging can improve the polyp detection rate(PDR).However,it is unknown whether optical enhancement in colonoscopy increases PDR when compared with high⁃definition white⁃light imaging(HDWL).Aims:To investigate the application value of optical enhancement endoscopy for colorectal polyp screening.Methods:A total of 371 patients underwent colonoscopy without sedation from August 2020 to November 2023 at Xingtai People′s Hospital were retrospectively analyzed.According to the mode during withdrawal,patients were divided into HDWL group and optical enhancement group.PDR and adenoma detection rate(ADR)were compared between the two groups.Results:A total of 371 patients underwent colonoscopy were enrolled in this study,including 199 in HDWL group and 172 in optical enhancement group.No significant differences in gender,age and bowel preparation score were found between HDWL group and optical enhancement group(P>0.05).The PDR(57.0%vs.42.2%,P=0.005)and ADR(25.6%vs.14.6%,P=0.008)were significantly higher in optical enhancement group than in HDWL group.Gender subgroup analysis showed that PDR(65.9%vs.45.9%,P=0.007)and ADR(31.8%vs.17.3%,P=0.023)in male patients in optical enhancement group were significantly higher than those in HDWL group.Conclusions:Routine use of optical enhancement mode can significantly improve the PDR and ADR during colonoscopy.
