移行型脑膜瘤术后恶性转化伴肋骨转移1例

摘　　要：脑膜瘤是中枢神经系统中最常见的原发性肿瘤之一,来源于中枢神经系统的蛛网膜细胞或脑膜上皮细胞。值得注意的是,良性脑膜瘤术后颅外转移极为罕见,以前仅被认为与恶性脑膜瘤有关,因其具有侵袭性,预后较差。随着研究者们对脑膜瘤了解的不断深入,发现部分良性脑膜瘤也具有恶性潜能,但目前尚罕见关于移行型脑膜瘤术后恶变并肋骨转移的报道。笔者报道1例8年前完全切除的移行型脑膜瘤术后复发、恶性转化为非典型脑膜瘤且转移到肋骨的病例,以及诊疗结果。Meningioma is one of the most common and frequent primary tumors of central nervous system,and it originates from either arachnoid or meningothelial cells in central nervous system.The extracranial metastases following surgery of benign meningioma are rare,and it was previously considered that these were only associated with malignant meningiomas which are aggressive and have a poor prognosis.With the increased understanding of meningioma obtained from previous studies,it is now known that some benign meningiomas also have malignant potential.However,there are currently no reports of postoperative malignant transformation with rib metastasis of transitional meningiomas.The present study reports the case of a patient with transitional meningioma,for which total resection was performed 8 years ago.Postoperative recurrence,malignant transformation to atypical meningioma,and metastases to ribs occurred in the patient.The diagnosis and treatment results were reported here.
