摘　　要：目的:探讨胃癌组织中甲胎蛋白(AFP)的表达和预后以及与免疫特征的相关性。方法:采用免疫组织化学法检测AFP在胃癌组织、癌旁组织、腺瘤组织中的表达,分析AFP表达与胃癌临床病理特征的关系及与预后的相关性,基因集富集分析AFP在胃癌进展中的潜在机制。结果:胃癌组织中AFP高表达37例,低表达26例,胃癌组织中AFP的表达显著高于癌旁组织、腺瘤组织,AFP高表达组的淋巴结转移和脉管侵犯比例均显著高于低表达组(χ^(2)=2.020、6.293,P<0.05)。预后分析结果显示AFP高表达组的住院时间显著高于AFP低表达组(t=3.816,P<0.05),总生存期显著低于AFP低表达组(t=6.165,P<0.05)。胃癌中高表达的AFP主要富集在上皮细胞-间充质转化(EMT),并且与SNAIL1、STAT5、KRAS为正相关(r=0.550、0.429、0.221,P<0.05),并且与PD-L1、PD-L2、LAG3、CTLA4和TIM3等免疫治疗检查点基因均呈正相关(r=0.373,0.643,0.233,0.271,0.620,P<0.05)。结论:AFP在胃癌组织中表达上调,可能通过EMT和免疫检查点等途径促进胃癌发展。Objective:To investigate the expression and prognosis of alpha-fetoprotein(AFP)in gastric cancer and its correlation with immune characteristics.Methods:Immunohistochemistry was used to detect the expression of AFP in gastric cancer tissues,adjacent tissues,and adenoma tissues.The correlation between AFP expression and clinicopathological features of gastric cancer and prognosis was analyzed.The potential mechanism of AFP in the progression of gastric cancer was analyzed by gene enrichment analysis.Results:AFP expression in gastric cancer tissues was significantly high expression in 37 cases and significantly low expression in 26 cases.AFP expression in gastric cancer tissues was significantly higher than that in paracancerous tissue and adenoma tissue.The proportions of lymph node metastasis and vascular invasion in the AFP high expression group were significantly higher than those in the low expression group(χ^(2)=2.020,6.293,P<0.05).The results of prognosis analysis showed that the hospital stay in the high AFP expression group was significantly longer than that in the low AFP expression group(t=3.816,P<0.05),and the overall survival was significantly lower than that in the low AFP expression group(t=6.165,P<0.05).AFP was highly expressed in gastric cancer and mainly con-centrated in epithelial-mesenchymal transformation(EMT),which was positively correlated with SNAIL1,STAT5,and KRAS(r=0.550,0.429,0.221,P<0.05).It was positively correlated with PD L1,PD-L2,LAG3,CTLA4,TIM3 and other immunotherapy checkpoint genes(r=0.373,0.643,0.233,0.271,0.620,P<0.05).Conclusion:AFP expression is up-regulated in gastric cancer,which may promote the development of gastric cancer through EMT and immune checkpoint.
