海马型康复体位护理对卒中偏瘫患者的影响

摘　　要：目的分析海马型康复体位护理对卒中偏瘫患者肢体活动功能、生活质量及肌力水平的影响。方法选择2022年6月至2024年3月本院收治的脑卒中偏瘫患者80例,按照Excel对患者随机排序1~80号,奇数序列开展常规康复护理(40例,对照组),偶数序列开展海马型康复体位护理(40例,观察组)。针对两组患者肢体功能评分、生活质量、睡眠质量、肌力水平进行对比。结果护理后,观察组上肢运动功能Fugl-Meyer评定量表(FMA)评分、下肢功能FMA评分、改良Barthel指数较对照组高(P<0.05);观察组睡眠障碍阿森斯失眠量表(AIS)各维度评分(入睡延迟、夜间睡眠中断、早醒、白天困意)较对照组低(P<0.05);观察组世界卫生组织生存质量测定简表(WHOQOL-BREF)各维度(生理、环境因子、社会关系、心理)评分较对照组高(P<0.05);观察组三角肌、肱二头肌、臀大肌、股四头肌肌力水平均高于对照组(P<0.05)。结论在对卒中偏瘫患者护理时开展海马型康复体位护理,可以提升患者肢体活动能力以及肌力水平,增加患者在恢复期间生活质量,改善康复过程中睡眠质量,有助于患者恢复。Objective To analyse the effects of hippocampal rehabilitation position nursing on limb activity function,quality of life and muscle strength level of stroke hemiplegic patients.Methods A total of 80 cases of stroke hemiplegic patients admitted to our hospital from June 2022 to March 2024 were selected,and the patients were randomly sorted from No.1 to No.80 according to Excel,with conventional rehabilitation nursing carried out in odd-numbered sequences(40 cases,the control group),and hippocampal rehabilitation postural nursing carried out in even-numbered sequences(40 cases,the observation group).Comparisons were made for the limb function scores,quality of life,sleep quality,and muscle strength levels of the two groups.Results After nursing,the upper limb function Fugl-Meyer assessment(FMA)score,lower limb function FMA score,and modified Barthel index of the observation group were higher than those of the control group(P<0.05);the scores of the dimensions of sleep disorder Athens insomnia scale(AIS)(delayed sleep onset,nighttime sleep disruption,early awakening,and daytime drowsiness)of the observation group were lower than those of the control group(P<0.05);and the scores of the dimensions of WHO quality of life(WHOQOL-BREF)(physiological,environmental factors,social relations,psychological)scores were higher than those of the control group(P<0.05);and the muscle strength levels of the deltoid,biceps brachii,gluteus maximus,and quadriceps femoris muscles of the observation group were higher than those of the control group(P<0.05).Conclusions Carrying out hippocampal rehabilitation position nursing in the care of hemiplegic patients with stroke can improve the patients'limb mobility as well as the level of muscle strength,increase the patients'quality of life during the recovery period,improve the quality of sleep during the recovery process,and help the patients to recover.
