浓缩生长因子联合自体骨在颌骨囊肿治疗中的临床效果研究

摘　　要：目的 探讨浓缩生长因子联合自体骨在颌骨囊肿治疗中的临床应用效果。方法 选择2018年3月—2021年5月在嘉峪关市中医医院治疗的71例颌骨囊肿患者,按骨腔内充填物及创面覆盖物不同分为研究组(n=42)和对照组(n=29),其中研究组患者骨腔内填入浓缩生长因子、自体骨及Bis-Oss骨粉的混合物,创面覆盖浓缩生长因子膜及人工生物膜;对照组患者骨腔内只填入Bis-Oss骨粉,创面覆盖单层人工生物膜。观察两组患者术中出血量、术后肿胀及疼痛、切口拆线时间、伤口软组织愈合情况,及骨缺损区成骨情况,对比术后1、2周,1个月时瘢痕评分,术后3、6、12个月平均骨密度亨氏单位、角化龈宽度。结果 研究组患者术中出血量、术后肿胀及疼痛、切口拆线时间,及术后组瘢痕评分均较对照组患者明显降低(P<0.05),术后6、12个月研究组患者平均骨密度亨氏单位、角化龈宽度均较对照组明显升高(P<0.05)。研究组患者治疗总有效率优于对照组。结论 浓缩生长因子联合自体骨在颌骨囊肿摘除术后创口愈合过程中具有促进牙龈软组织修复再生、减轻瘢痕组织、缩短骨缺损愈合时间及诱导成骨的作用,是一种新型、有效的颌骨囊肿手术治疗方式,具有临床应用推广价值。Objective To explore the clinical effect of highly concentrated growth factor combined with autogenous bone in the treatment of apical cyst.Methods 71 patients with jaw cyst who were treated in Jiayuguan Traditional Chinese Medicine Hospital from March 2018 to May 2021 were selected and divided into a study group(n=42) and a control group(n=29) according to different intraosseous fillings and wound coverings.In the study group,a mixture of concentrated growth factor,autogenous bone and Bis-Oss bone powder were filled into the bone cavity.The wound was covered with concentrated growth factor membrane and artificial biofilm.In the control group,only Bis-Oss bone powder was filled into the bone cavity,and the wound was covered with a single artificial biofilm.Intraoperative blood loss,postoperative swelling and pain,incision removal time,wound soft tissue healing and bone formation in the bone defect area were observed in the two groups.Scar scores at 1 week,2 weeks and 1 month after surgery,average bone density HU and keratinized gingival width at 3 months,6 months and 12 months after surgery were compared.Results The amount of blood loss,swelling and pain,incision removal time and scar score in the study group were significantly decreased compared with the control group(P < 0.05).The average bone density HU and keratinized gingiva width in the former were significantly increased at 6 months and 12 months after the operation(P < 0.05).Its total effective rate was better than that of the control group(P < 0.05).Conclusion Concentrated growth factor combined with autogenous bone can promote the repair and regeneration of gingival soft tissue,reduce scar tissue,shorten the healing time of bone defect and induce bone formation in the process of wound healing after jaw cyst extraction.This provides a new and effective surgical treatment of jaw cyst,which is worthy of clinical application and promotion.
