三维后装与二维后装对宫颈癌患者膀胱、乙状结肠及直肠受照剂量的影响

摘　　要：目的探讨三维后装与二维后装对宫颈癌患者膀胱、乙状结肠及直肠受照剂量的影响。方法将55例宫颈癌患者,按照后装治疗技术分为两组,二维组(25例)、三维组(30例),所有患者均接受调强外放疗及顺铂同步化疗,二维组进行二维后装放疗,三维组进行三维后装放疗,观察比较两组临床疗效、直肠和膀胱受照剂量,放疗急性不良反应。结果三维组临床疗效有效率(96.67%)较二维组(80.00%)高(P<0.05);三维组患者治疗后膀胱和直肠受照剂量均低于二维组(P<0.05);三维组放疗急性直肠炎、急性膀胱炎、乙状结肠炎发生率分别为76.67%、10.00%和3.33%,均较二维组(96.00%、32.00%、20.00%)低(P<0.05)。结论三维后装放疗相比于二维后装放疗,临床治疗效果更好,且对直肠、膀胱危及器官照射剂量更低,急性放疗不良反应发生率更低。Objective To discuss the effect of radiotherapy using three-dimensional vs two-dimensional afterloading technique on radiation dose to bladder,sigmoid colon and rectum in patients with cervical cancer.Methods 55 patients with cervical cancer were divided into 2 groups according to post-loading treatment techniques:2D group(25 cases)and 3D group(30 cases).All patients received intensity-modulated external radiotherapy and cisplatin synchronous chemotherapy,2D group received 2D post-loading radiotherapy,and 3D group received 3D post-loading radiotherapy.Clinical efficacy of the 2 groups was observed and compared.Rectal and bladder irradiation doses,acute adverse reactions to radiotherapy.Results The clinical efficacy rate of 3D group(96.67%)was higher than that of 2D group(80.00%)(P<0.05).The doses received to bladder and rectum after treatment in 3D group were lower than those in 2D group(P<0.05).The incidence rates of acute proctitis,cystitis and sigmoiditis in the three-dimensional group were 76.67%,10.00%and 3.33%,respectively,lower than those in the two-dimensional group(96.00%,32.00%and 20.00%),P<0.05.Conclusion Compared with 2D posterior radiotherapy,3D posterior radiotherapy has better clinical therapeutic effect and lower dose to the dangerous organs of rectum and bladder,so as to reduce the acute adverse reactions after radiotherapy.
